Changing Health Care Systems and Rheumatic Disease by Institute of Medicine,Committee on Changing Health Care Systems and Rheumatic Disease Pdf

Market forces are driving a radical restructuring of health care delivery in the United States. At the same time, more and more people are living comparatively long lives with a variety of severe chronic health conditions. Many such people are concerned about the trend toward the creation of managed care systems because their need for frequent, often complex, medical services conflicts with managed care's desires to contain costs. The fear is that people with serious chronic disorders will be excluded from or underserved by the integrated health care delivery networks now emerging. Responding to a request from the National Institute of Arthritis and Musculoskeletal and Skin Diseases, this book reflects the results of a workshop that focused on the following questions: Does the model of managed care or an integrated delivery system influence the types of interventions provided to patients with chronic conditions and the clinical and health status outcomes resulting from those interventions? If so, are these effects quantitatively and clinically significant, as compared to the effects that other variables (e.g., income, education, ethnicity) have on patient outcomes? If the type of health care delivery system appears to be related to patient care and outcomes, can specific organizational, financial, or other variables be identified that account for the relationships? If not, what type of research should be pursued to provide the information needed about the relationship between types of health care systems and the processes and outcomes of care provided to people with serious chronic conditions?

EBOOK: Caring for People with Chronic Conditions: A Health System Perspective by Ellen Nolte,Martin McKee Pdf

"Overall, the book walks a delicate balance between evidence and advocacy regarding the care of people with chronic conditions. Nolte and McKee conclude the volume with the following: 'A first step is to recognize that something must be done. A second, which we hope will be facilitated by the evidence provided in this book, is to realize that something actually can be done, and that they can do it (p. 240)'. The overarching desire to match the need for evidence with the reality that advocates (including policy-makers) need a reasoned voice makes the book well suited to health policy deliberations." International Journal of Integrated Care The complex nature of many chronic diseases, which affect people many different ways, requires a multifaceted response that will meet the needs of the individual patient. Yet while everyone agrees that the traditional relationship between an individual patient and a single doctor is inappropriate, there is much less agreement about what should replace it. Many countries are now experimenting with new approaches to delivering care in ways that do meet the complex needs of people with chronic disorders, redesigning delivery systems to coordinate activities across the continuum of care. Yet while integration and coordination have an intuitive appeal, policy makers have had little to help them decide how to move forward. The book systematically examines some of the key issues involved in the care of those with chronic diseases. It synthesises the evidence on what we know works (or does not) in different circumstances. From an international perspective, it addresses the prerequisites for effective policies and management of chronic disease. Taking a whole systems approach, the book: Describes the burden of chronic disease in Europe Explores the economic case for investing in chronic disease management Examines key challenges posed by the growing complexity in healthcare including prevention, the role of self-management, the healthcare workforce, and decision-support Examines systems for financing chronic care Analyses the prerequisites for effective policies for chronic care Caring for People with Chronic Conditions is key reading for health policy makers and health care professionals, as well as postgraduate students studying health policy, health services research, health economics, public policy and management. Patient Centered Medicine by Omur Sayligil Pdf

Patient-centered medicine is not an illness-centered, a physician-centered, or a hospital-centered medicine approach. In this book, it is aimed at presenting an approach to patient-centered medicine from the beginning of life to the end of life. As indicated by W. Osler, "It is much more important to know what sort of a patient has a disease than what sort of a disease a patient has." In our day, if the physicians and healthcare professionals could consider more than the diseased organ and provide healthcare by comforting the patients by respecting their values, beliefs, needs, and preferences; informing them and their relatives at every stage; and comforting the patients physically by controlling the pain and relieving their worries and fears, patients obeying the rules of physicians would become patients with high adaptation and participation to the treatment.

Managing Chronic Illness Using the Four-Phase Treatment Approach by Patricia A. Fennell Pdf

A pioneering book to help maximize the quality of life for chronically ill patients Written by a leading authority on chronic illness treatment and management, Managing Chronic Illness Using the Four-Phase Treatment Approach provides evidence-based practice guidelines for clinicians to help their clients with debilitating health problems embrace a new "normal," understand the cyclical nature of their illness, and function at the highest level possible. Patricia Fennell's groundbreaking model for understanding chronic illness identifies and describes four broad phases experienced by the chronically ill: crisis, stabilization, resolution, and integration. Using a broad array of case histories, Fennell vividly illustrates what clients need at each phase and how to assess and respond to them compassionately. Fennell also suggests how clinicians may best use their own changing experiences in their work to help clients transition through the four phases. The goal of the "Four-Phase Model" is to maximize a client's quality of life without offering false hope for a cure, making it an effective treatment strategy for diverse client populations, including people with physiological diseases; patients whose lives are being prolonged by modern medicine; and people who suffer from addiction, post-traumatic stress syndrome, intractable pain, and post-rape and abuse conditions. Complete with detailed treatment protocols for documenting a client's symptoms and quality of life at each phase, Managing Chronic Illness Using the Four-Phase Treatment Approach is a highly practical book for everyone working with chronically ill clients.

Living Well with Chronic Illness by Institute of Medicine,Board on Population Health and Public Health Practice,Committee on Living Well with Chronic Disease: Public Health Action to Reduce Disability and Improve Functioning and Quality of Life Pdf

In the United States, chronic diseases currently account for 70 percent of all deaths, and close to 48 million Americans report a disability related to a chronic condition. Today, about one in four Americans have multiple diseases and the prevalence and burden of chronic disease in the elderly and racial/ethnic minorities are notably disproportionate. Chronic disease has now emerged as a major public health problem and it threatens not only population health, but our social and economic welfare. Living Well with Chronic Disease identifies the population-based public health actions that can help reduce disability and improve functioning and quality of life among individuals who are at risk of developing a chronic disease and those with one or more diseases. The book recommends that all major federally funded programmatic and research initiatives in health include an evaluation on health-related quality of life and functional status. Also, the book recommends increasing support for implementation research on how to disseminate effective longterm lifestyle interventions in community-based settings that improve living well with chronic disease. Living Well with Chronic Disease uses three frameworks and considers diseases such as heart disease and stroke, diabetes, depression, and respiratory problems. The book's recommendations will inform policy makers concerned with health reform in public- and private-sectors and also managers of communitybased and public-health intervention programs, private and public research funders, and patients living with one or more chronic conditions.
