The purpose of this research project is threefold: to examine the current knowledge of state-of-the-art highway design practices aimed at increasing the safety of older drivers; to assess the crash and fatality data for older drivers in Arizona; and to survey older adults regarding their perceptions of Arizona's roadways and possible needs for enhancement. Older adults increasingly make up a larger part of the driving population. Age related declines and complications from medical conditions put older drivers at higher risk of collision, and when in collision, of a fatal injury. It is recommended that Arizona use locations identified in this study as having high rates of collisions involving older adults to develop test sites for roadway improvements. It is also recommended that the state begin to review its screening, assessment and education for older drivers with the intent of developing a more stringent screening and assessment process and develop and implement self-testing for older adults to support improved driving safety.

In a study of older adults and their travel patterns in Oregon, a statewide mail survey and telephone interviews were conducted with older drivers and older adults who had voluntarily chosen to stop driving. The purpose of the study was to determine: (1) the factors that influence driving cessation; (2) the physical and emotional barriers that delay driving cessation; (3) what opportunities exist for alternative transportation after driving cessation; (4) whether drivers make relocation decisions on the basis of driving cessation; (5) the warning signs that make a driver stop driving; and (6) whether a crisis situation generally forces a driver to stop driving. Completed mail surveys included those from 342 respondents who were current drivers (184 urban and 141 rural, plus 17 who did not report whether they lived in an urban or rural area) and 158 respondents who had voluntarily ceased driving (110 urban and 37 rural, plus 11 who did not report their urban/rural status). Telephone interviews were completed with 33 urban drivers, 36 rural drivers, 25 urban ceasers, and 6 rural ceasers. Changes in driving patterns, occurred gradually and late, mostly for respondents in their late 70's or early 80's. Among the results of the study was the finding that those most likely to have chosen to stop driving were older, depressed females in poorer health who were living in senior housing, using alternative transportation when available, making fewer trips, and seeing fewer limitations associated with using alternative transportation. Relocation to improve access to transportation alternatives was not seen by most respondents as a viable option. Because this study was a cross-sectional, not longitudinal, analysis, it was not possible to determine causality (e.g., to know whether people became depressed as a result of ceasing to drive or being depressed led them to cease driving, or to know whether poor health was the result or cause of driving cessation). Additional research is needed to establish causality, ideally following groups of individuals in particular age cohorts over time, including individuals forced to stop driving through the Medically-At-Risk Program. Such research would enable better understanding of the factors affecting driving decisions and the effects of those decisions, and help to signal possible strategies to implement to encourage the use of alternative transportation.

This one-of-a-kind comprehensive study highlights the importance of automated testing techniques and the significance of vision screening measures other than standard visual acuity testing for assessing all drivers and, in particular, at-risk drivers and older drivers. Non-automated tests tend to be subjective, time-consuming, costly, and heavily reliant on the experience of the examiner. Due to the high collision, injury, and fatality rates of all drivers in the State of Arizona, and the disproportionate number of at-fault older drivers and collision risks in the States of Arizona and Florida, new and automated screening methodologies and vision standards are now needed to promote road safety, predict visual impairment, and evaluate possible restriction or confiscation of driver's licenses. This study demonstrates that environmental factors and manner of collisions increase in collision involvement for drivers between ages 50 to 59 years in both Arizona and Florida. Drivers age 80 to 89 years in both states are most likely at-fault in collisions compared to all other age cohorts. These results are consistent among drivers cited for collision involvement due to visual defects. These findings, which span an 11-year period from 1991 to 2001, not only apply to Arizona and Florida, two states with some of the largest proportions of older individuals in the United States, but, as a global survey of motor vehicle bureau directors or their representatives in the United States, Commonwealth of Puerto Rico, United Kingdom, Canada, New Zealand, and Australia illustrate, any state, country, province, territory, commonwealth, or nation with an increasing number of older drivers. A pilot study, to follow, ultimately allows for the implementation of effective strategies for screening of visual impairment and eye disease in all Arizona drivers. Snellen acuity, the most widely used vision testing measure, accounts for less than 0.1% of the visual field and fails to quantify contrast sensitivity and color vision (Fink and Sadun, 2004), two of several visual parameters needed for safe driving. It is recommended that at-risk and older drivers in Arizona be tested for vision through a newly designed system of measures provided by two automated tests (to test vision condition and function) and one driving simulator (to assess eye status). Hence, it is integrated into a larger system and additional recommendations are provided as these relate to motor vehicle operation skills and cognition. These automated systems and methodologies may ultimately serve as a prototype of transportation license testing improvements for all other states, countries, and agencies (e.g., aviation, rail, maritime, commercial vehicles, etc.) to follow. Such techniques may also reduce the incidence of fraudulent schemes and issuances of driver's licenses, commercial driver's licenses, and hazardous materials transportation licenses.
