The Authors object has been to place in the hands of his readers an epitome of the science and practice of midwifery which embodies all recent advances. He is aware that on certain important points he has recommended practice which not long ago would have been considered heterodox in the extreme, and which, even now, will not meet with general approval. Excerpt from A Treatise on the Science and Practice of Midwifery During the intervening four years since the last American edition was issued, very decided advances have been made in the field of obstetric surgery, particularly in the adoption of methods that have resulted in the saving of human life. The conservative Caesarean operation in the most carefully managed European maternities, and even in the general practice in our own country, may be honestly claimed to save 90 per cent. of the women - 16 having died out of 160 cases in certain European hospitals, and 2 out of the last 20 in the United States. The Porro-Caesarean record of all countries now shows a mortality which has been reduced to 14 per cent., as proven by the record of 1890-1891; and the sub-peritoneal, replacing the fatal intra-peritoneal method, has saved 22 women out of 25. The introduction of symphyseotomy into our country a year ago, has required us to give a special notice to this operation, now so largely performed in Europe, and attracting a growing attention here, because of its possibilities of success. The term laparotomy, and the prefix laparo-, as applied to abdominal surgery, and not to flank-incisions, have been abandoned throughout the volume, and the term c diotomy, and the prefix c lio-, substituted for them. Craniotomy having fallen in the estimation of American obstetricians, and some of the younger thinkers of Great Britain, because of the diminishing dangers of Caesarean and symphyseotomic deliveries, the American editor has striven to do away with the leaning of the English author toward a preference for the infantile destructive method, begotten of opinions based upon comparative results which were prevalent, and thought to be well founded, forty years ago. Excerpt from A Text Book of Midwifery, Vol. 1 AT the outset of my career as teacher and practitioner, nearly twenty years ago, I wrote a Compendium of Midwifery which the publishers intended should form one of a series of text books, and which met with a far more cordial reception than I had anticipated. During the next few years however of my clinical activity, I had neither time nor inclination for a more complete and revised treatise, and before long my removal to another sphere of work, which happened more than once, also proved an obstacle. When later on I found the necessary leisure, the need for a Compendium had passed away. It seemed to me how ever that a complete treatise of Obstetrical Science, showing what progress had been made both in theory and practice during the last twenty years, and how greatly the science had been advanced by fresh researches, would be a useful undertaking. Especially did I venture to hope that the wide gap which still separates the Midwifery as taught in our hospitals and medical schools, and that of ordinary practice might be partly bridged over. Such' was the origin of this text book. My last object obliged me to treat the subject from a clinical stand-point. Excerpt from The Science and Art of Midwifery The third edition of this work, published in 1885, was intended to present to the reader a picture of the most advanced obstetrical teachings of that date. In the brief interval, however, that has elapsed since then the changes that have taken place in both the theory and practice of obstetrics have made it necessary for me to present to the profession what is essentially a new book. Thus many modifications in the theory have resulted from more careful observations in anatomy and pathology, and from fruitful physio logical investigations. It has been my endeavor to interweave aseptic precautions with all branches of obstetric art without, how ever, insisting upon pedantic measures which experience has shown to be needless. But the noblest conquests of the past years have been the result of the employment of modern surgery for the relief or removal of a host of complications formerly regarded as beyond the pale of human help. In making needed alterations I have not felt it obligatory when new discoveries have destroyed the value of former deductions to retain these in order to maintain a reputation for consistency. I thank the medical press and the medical profession of this country and Great Britain for the flattering welcome they have hitherto extended to this treatise, and trust that in its new form it may command a like degree of favor.
