Most patients in developed nations with medical problems requiring hospital care are elderly. Increasingly the dividing line between general internal medi cine and acute geriatric medicine is becoming more blurred. It is, nevertheless, apparent that some elderly patients on medical or sub-specialty hospital wards become 'bed blockers'. Why? Also, why are 'bed blockers' less of a problem on an acute geriatric ward? Many clinicians believe this is related to a faster access to the long-stay beds of the geriatric unit. Even a brief study of hospital operating statistics will show this is not and cannot be the case. When geria tricians are asked to see elderly 'bed blockers' on colleagues' wards they approach with anxiety because these patients often have to be placed on a long waiting list for these scarce and very expensive continuing care beds. Do geria tricians see different acute medical problems compared with their colleagues? The answer is not immediately obvious, though geriatricians are likely to receive more potential 'bed blockers' than their general medical colleagues. How is it then, that geriatricians seem to cope better than their colleagues? All geriatricians have experience of general internal medicine but the opposite unfortunately does not hold. This book is written in the hope of redressing the imbalance.

Care for older adults in Emergency Departments (EDs) has historically been focused on acute medical conditions with little emphasis on geriatric-specific issues. In 2010, emergency departments (EDs) throughout the nation saw almost 130 million patients, 15% of whom were 65 or older. The number of older adults who visit an ED has doubled in the last decade and continues to grow rapidly. Older adults receiving care in an ED are highly likely to be admitted to the hospital, much more so than their younger counterparts. Preventing a hospital admission saves older adults from frequently encountered adverse events, including hospital-acquired delirium, functional status impairment, cognitive loss, and nursing home admission. It is unknown how many older adults are hospitalized for reasons other than acute medical illness, such as functional decline, polypharmacy, progressive dementia, caregiver stress, or unstable living situation. These non-emergent conditions are rarely addressed during a typical ED visit due to lack of resources, significant patient volumes, and the need for rapid turnover of care spaces. The predominant management strategy of emergency physicians to handle these important but not imminently life-threatening geriatric issues is to recommend hospital admission. Northwestern has pioneered the Geriatric Emergency Department through the creation of the Geriatric Emergency Department Innovation model (GEDI), with goals to prevent admissions for older adults by assessing and meeting their geriatric-specific, non-acute care needs in the ED. The GEDI model at Northwestern centers on a multi-disciplinary curriculum composed of clinical, didactic, and practical arms developed by emergency medicine and geriatrics educational experts. In this title, we will present case studies of older adults seen in the Emergency Department through the GEDI model and discuss means of identifying/screening for, diagnosing, and treating geriatric syndromes seen in the emergent care of the older adult patient. In addition, there will be a set of concise “take-home points” for each case study that will be easy to commit to memory and implement in clinical care of older adults. As the number of seniors seeking emergent care will continue to increase, the ED setting must become responsive to geriatric-specific needs. With the appearance of a textbook as comprehensive as this one, it is clear that the field of geriatrics is coming of age. The broad scope of these volumes shapes a substantial answer to the question, "What is geriatrics and why should we be interested in it?" As I see it, there are at least five reasons. First, the scientific or intellectual reason: gerontology is the study of aging from the biologic, psychological, and social perspectives. There is increasing interest in the fascinating insights into the biologic mechanisms of aging, errors in protein synthesis, DNA repair mechanisms, alterations of the neuroendocrine system, changes in the immune system, genetic controls, and somatic mutations. Second, the demographic reason: this is the century of old age. There has been a 26-year gain in the average life expectancy. This gain compares with that acquired from 3,000 years B.C. (the Bronze Age) to the year 1900, which was about 29 years. Therefore, in one century, there has been a gain in the average life expectancy almost equal to 5,000 previous years of human history. In 1830, one of three newborn infants survived beyond 60 years of age.
