Case Studies of Postoperative Complications after Digestive Surgery by Miguel A. Cuesta,H. Jaap Bonjer Pdf

The proposal for this book is to offer a systematic description of the most frequent complications occurring in the three parts of the digestive tract: HPB, Upper GI and colorectal tracts. Every complication, from esophageal to the rectum, is described systematically through two or three practical cases as has been treated by actual surgical practices of authors serving as surgeon practitioners. Description of the case, presentation of indication for surgery, type of primary surgical intervention and complication is described textually but also and by means of clinical signs, laboratory tests, radiological studies (CT scans and schematic drawings) and other methods used for diagnosis and treatment. The reader will have access to a practical book in which every current complication can be easily recognized, along with relevant information as guide for an adequate treatment.

Treatment of Postoperative Complications After Digestive Surgery by Miguel A. Cuesta,H. Jaap Bonjer Pdf

Any surgical intervention, elective or acute, may lead to postoperative complications. Moreover, the pertaining approach - laparoscopic or open - will probably not differ in the morbidity rate after surgery. Complications that occur after a surgical intervention can be classified as major or minor. Major complications to the digestive tract after surgery imply in most of cases a leakage of an anastomosis, bleeding in the abdominal cavity or in the tract, the appearance of intraperitoneal abscesses, or surgical site infections and wound dehiscence. These complications, depending on the organ affected, upper GI, HPB tract or colorectal, are associated with high morbidity and mortality. Early suspicion and diagnosis, followed by an early and effective treatment is imperative in order to reduce the morbidity and mortality. Adequate treatment will involve a good coordination of the three disciplines involved in treatment: the surgeon, the intervention radiologist, and the gastroenterologist. Approach of these postoperative complications is changing constantly and these changes are not properly known by general and more specialized digestive surgeons daily involved in the treatment of these diseases. The proposal for this book is to offer a systematic description of the most frequent complications occurring in the three above mentioned parts of the digestive tract. In this way, the reader will have access to a practical book in which every current complication can be easily recognized, along with relevant information as guide for an adequate treatment.

Post-operative Complications by David Leaper,Iain Whitaker Pdf

Complications following surgical procedures are not only associated with significant morbidity and mortality, but also add immensely to the psychological burden of the patient and are disappointing for the surgical team. Recent research has led to a greater understanding of the response to surgery and anaesthesia, and great advances have been made in pre-operative preparation with appropriate investigations and pharmacological prophylaxis. These have reduced the incidence of many complications, but the risks are impossible to remove completely. This wide-reaching and practical handbook covers all aspects of post-surgical complications. It is divided into two sections: the first covers general complications such as pressure sores, complications of fluid balance and problems related to anaesthesia, while the second focuses on the complications encountered in the major surgical specialties. Fully revised and updated since the previous edition, and written by a team of experts, Post-operative Complications is essential reading for all those involved in caring for surgical patients. It is particularly relevant for junior doctors and those preparing for their membership of the Royal College of Surgery (MRCS) exams.

Management of Gastric Cancer by Paul H. Sugarbaker Pdf

Gastric cancer has been one of the great malignant scourges affecting man kind for as long as medical records have been kept. Until operative resection pioneered by Bilroth and others became available, no effective treatment was feasible and death from cancer was virtually inevitable. Even with resection by total gastrectomy, the chances of tumor eradication remained small. Over recent years, however, the situation has been changing. Some changes have resulted from better understanding of the disease, early detec tion, and better management techniques with applied clinical research, but the reasons for other changes are poorly understood. For example, the incidence of gastric cancer is decreasing, especially in westernized societies, where it has fallen from one of the most common cancers to no longer being in the top five causes of cancer death. Still it remains the number one killer of adult males in Japan and Korea. Whether the reduced incidence in western societies is a result of dietary changes or methods of food preservation, or some other reason, is as yet uncertain. Improvements in outcome have been reported from mass screening and early detection; more refined techniques of establishing early diagnosis, tumor type, and tumor extent; more radical surgical resection; and resection at earlier stages of disease.

Pancreatic Cancer by Shubham Pant Pdf

Pancreatic cancer is a challenging and complex disease characterized by a dense fibrotic stroma and an immunosuppressive tumor microenvironment. Until recently, there were no predictive biomarkers to personalize selection of targeted or biologic therapies as a part of standard of care treatment. Moreover, the regulatory immune population of cells creates a “cold”, non-immunogenic tumor that is resistant to immunotherapies including checkpoint inhibitors. However, in the last few years, there has been substantial progress in our understanding of the role mutations in the pathogenesis of disease leading to increased adoption of germline and next generation sequencing to identify mutations and fusions to optimize selection of therapies and select patients for clinical trials of targeted agents. This book provides a comprehensive, global overview on therapy for pancreatic cancer, exploring approved therapies and focusing on the “Next” in drug development including molecularly targeted therapy and efforts at “Drugging the Undruggable”: the KRAS mutation. It also addresses efforts at targeting the inhospitable stroma to improve drug delivery to the tumor cells, incorporating ct DNA (liquid biopsies) in the care of patients and recent advances in immunotherapy. Pancreatic Cancer: Current Therapeutics and Future Directions will illuminate these challenges, review existing therapeutics, and highlight current and future efforts to improve outcomes in this devastating disease. Useful to physicians, fellows, medical students, residents, physician assistants, nurse practitioners, pharmaceutical companies and researchers interested in pancreatic cancer, it will first explain management of localized disease including resectability and adjuvant and neoadjuvant therapies. Targeted therapies including molecular, immunotherapy, stroma and role of ctDNA or “liquid biopsies” will be addressed. Finally, the book will explore the important role of pain management, diet and exercise in improving outcomes in pancreatic cancer.
