Child health in the United States is shamefully behind nearly every other economically and industrially advanced country. Contradictions of capability versus performance; Attitudes versus performance; programs versus performance; and intention versus results are evident. Sources of failure are 1) neglected social needs of children; 2) insufficient preventative services; 3) inadequate curative services; 4) administrative impediments; and 5) financial impediments. Reasons for failure are inherent in the system. Flaws in operation needs repair or replacement of parts. The successful programs of Holland, Scotland and other European countries can be adapted for the United States. It is recommended that the United States' outlook be restructured to 1) organize community child health programs administered by government; 2) establish children's health insurance for curative services. A trial by states is suggested.

Volume 1 summarizes the major findings and recommendations; Volume 2 outlines the analysis and recommendations for selected federal programs (Title V of the Social Security Act; Medicare and EPSDT; WIC program; the Handicapped Children Education Act; community health pr ograms; and major federal maternal and child health programs); Volume 3 presents a statistical profile of the most compr ehensive compilation of data on child health in the US to date; Volume 4, Part 1 comprises a set of commissioned papers prepared for working groups of the Select Panel for the Promotion of Child Health concerning health care and health poli cy toward childrenand pregnant women; and Volume 4, Part 2 presents a detailed comprehensive description regarding the f inancing and organization of federal health care services. The recommendations of the overall report reflect a realistic analysis of serious unmet needs in child and maternal health, a recognition of past sucesses and future challenges for meeting these needs, a detailed consideration of the weaknesses and strengths of current federal programs and policies, and a pragmatic assessment of the capacity of the US to provide parents and health care professionals the scientific, fi nancial, and organizational support required to improve child health. Increasing numbers of evidence-based interventions have proven effective in preventing and treating behavioral disorders in children. However, the adoption of these interventions in the health care system and other systems that affect the lives of children has been slow. Moreover, with few exceptions, current training in many fields that involve the behavioral health of children falls short of meeting the needs that exist. In general, this training fails to recognize that behavioral health disorders are among the largest challenges in child health and that changing cognitive, affective, and behavioral health outcomes for children will require new and more integrated forms of care at a population level in the United States. To examine the need for workforce development across the range of health care professions working with children and families, as well as to identify innovative training models and levers to enhance training, the Forum on Promoting Children's Cognitive, Affective, and Behavioral Health held a workshop in November 2016. Workshop panelists and participants discussed the needs for workforce development across the range of health care professions working with children, youth, and families, and identified innovative training models and levers for change to enhance training. This publication summarizes the presentations and discussions from the workshop.
Primary Care now highlights two additional areas compared to the previous edition, equity in health services and health, and the overlap between clinical medicine and public health. It provides a basis for future directions in health policy.

One understanding of child maltreatment is limited in that it is based almost entirely on research and clinical experience in Western nations. The cross-cultural record, a "natural laboratory" of human behavior, allows a consideration of child abuse and neglect from the perspective of a wider range of social and environmental conditions. Each of the nine original essays in this volume examines child-rearing practices and child maltreatment in the context of a culture very different from our own. There is no universal standard for optimal child rearing, nor for child abuse and neglect. Seeking culturally appropriate definitions of child abuse, the authors stress the socialization goals of the particular cultural group, the intentions and beliefs of adults in the group, and the interpretations children place on their treatment. The authors differentiate practices such as harsh initiation rites, severe punishment, or, conversely, many Western practices viewed as abusive by other cultures, from idiosyncratic mistreatment by individuals. They further distinguish idiosyncratic child abuse and neglect form the suffering caused children, and their families, by circumstances such as poverty, food scarcity, and disease. Though several of the essays focus on the socioeconomic factors implicated in the etiology of child abuse (particularly rapid socioeconomic change), they indicate that cultural factors determine how a society will respond to negative socioeconomic conditions. The authors concur that while children may be exposed to considerable hardship in these non-Western societies, harm at the hands of individual caretakers is rare. They consider factors in the cultural context that may act either to increase or to decrease the likelihood of child abuse and neglect. Among these factors are the value that a culture places on children in general, categories of children that are more vulnerable to mistreatment, beliefs about the developmental age capabilities of children, and, most important, the embeddedness of child rearing in a network of kin and community that extends beyond individual biological parents.
