After-visit reports must be filed, future daily visits must be scheduled, and things like mileage and out-of-pocket expenses must be logged.This unique planner/organizer is designed to help the home-health nurse be as efficient as possible by capturing all of the details necessary for reporting while also providing prompts for conducting the best possible assessment of each patient. Logically organized, it is divided into 12 monthly segments followed by 60 blank two-page assessments. Each month begins with a two-page spread comprised of a full calendar to the left and a log of all patients seen during that particular month on the right.
