Massive experimental, computational, and clinical studies have been performed worldwide, and are still ongoing, to understand and characterize Covid-19 molecular basis and transmission mechanisms, to develop diagnostics and vaccines, and to search for small chemical drug candidates and therapeutic proteins and peptides. Impressive results have been obtained for transmission control and vaccines so far, but what is the status of the other therapeutic options? The crisis has exposed different types of weaknesses in biomedical research in many countries. What can we learn from this crisis in the field of drug discovery and development so as to emerge stronger? The Covid-19 crisis has revealed the strengths of modern drug discovery and vaccine development but also exposed different types of weaknesses that would need to be addressed to be better prepared for a possible next global health crisis. These challenges/weaknesses/obstacles are of very different nature and as such hard to tackle. For instance, in addition to the inherent challenging nature of the scientific discovery, some scientists have mentioned insufficient local/national coordination and observed a fragmented drug discovery research, others have underlined a lack of coordination between the academic system and the private sector, an inadequate international global coordination and cooperation, others highlighted insufficient infrastructures, inappropriate financial supports, limited discussions between the scientific community performing the research and the public health authorities, decision makers and the society, while some remarked insufficient education/training about drug discovery and development leading to confusion… The crisis also raised important scientific questions about the technologies that would need to be used during an emergency situation, combined, integrated, or developed so as to accelerate the identification of small molecule drug candidates and of therapeutic peptides/proteins.

SARS is a newly identified human infection caused by a corona virus unlike any other known human or animal virus in its family. The analysis of epidemiological information obtained from the sites of the outbreaks of SARS is still underway but the overall case fatality ratio is known to approach 11% although the rate among the elderly is much higher. Currently the major challenges for the treatment of SARS are: the source of the SARS virus and mode of transmission are still not well understood; there are problems with diagnostic tools; there is no effective treatment; and there is no vaccine for SARS. The above-mentioned difficulties and challenges have motivated national authorities health workers and scientists to explore the potential of complementary treatment. The results of research on integrated treatment with TCM and Western medicine showed that it is safe and that it also has some potential clinical benefits. Therefore the experts suggested that records of such experience could serve as reference material for treatment of SARS in the future. Interest in the coronaviruses has never been greater. Their economic impact is considerable as they infect humans, livestock, poultry and companion animals. Murine hepatitis virus (MHV) infection of the mouse and rat central nervous systems are the subject of intense study; these investigations are providing insights into the potential role of viruses in human neurological diseases and, more generally, into mechanisms causing neurological damage. The single-stranded, positive-sense RNA genomes of two species of these enveloped viruses (IBV and MHV) have been cloned completely and one of them (lBV) sequenced in its entirety, revealing a genome size of some 27000 nucleotides. This has made possible more incisive investigations into the nature of those polypeptides, encoded by more than half of the genome, which are likely to contribute, in the main, to RNA polymerase/replicase activity. Intriguingly, ribosomal frameshifting is exhibited within the mRNA coding for these polypeptides. The cloning/sequencing phase of coronavirology for which the 1980's will be partly remembered, has provided a sound framework for furthex: studies of the virus structural proteins and also some provocative insights relevant to these studies. The large spike glycoprotein(s), responsible for membrane fusion and bearing important antigenic sites, varies amazingly in length and composition both within as well as between coronavirus species. Receptors on host cells have been identified. The integral membrane glycoprotein (M) has been shown to use internal hydrophobic sequences to direct translocation within membranes.

Least developed countries and countries in the process of developing the health components of National Adaptation Plans (NAPs) under the UN Framework Convention on Climate Change (UNFCCC) (4) may find this document particularly useful in their efforts to design a comprehensive response to the risks presented by short-term climate variability and long-term climate change.
