Most medical schools in the US, Canada and UK now incorporate some form of arts and humanities-based teaching into their curricula. What happens in residency is another story. Most postgraduate programs do not continue the thread of such teaching although many residents would like to deepen their understanding of the medical humanities before they move into practice. The humanities emphasize "the human side of medicine", and can provide a counterpoint to the reductionism of evidence-based medicine and technological hubris for young doctors as they apply new knowledge and skills in ambiguous, real-life encounters with patients who are living with complicated health problems. Humanities-based education can help both sides of the relationship: programs are shown to reduce burnout and mental health issues in young physicians, and can also help learning practitioners grapple with the most difficult aspects of their craft: how does one persuade patients on a course of treatment, while respecting informed consent? How does one work with families? How does one listen to and treat patients exhibiting self-harm tendencies? During the apartheid era, thousands of South African political activists, militants, and refugees fled arrest by crossing into neighbouring southern African countries. Although they had escaped political oppression, many required medical attention during their period of exile. An Ambulance on Safari describes the efforts of the African National Congress (ANC) to deliver emergency healthcare to South African exiles and, in the same stroke, to establish political legitimacy and foster anti-apartheid sentiment on an international stage. Banned in South Africa from 1960 to 1990, the ANC continued its operations underground in anticipation of eventual political victory, styling itself as a "government in waiting." In 1977 it created its own Health Department, which it presented as an alternative medical service and the nucleus of a post-apartheid healthcare system. By publicizing its own democratic policies as well as the racist practices of healthcare delivery in South Africa, the Health Department won international attention for its cause and provoked widespread condemnation of the apartheid state. While the global campaign was unfolding successfully, the department's provision of healthcare on the ground was intermittent as patients confronted a fledgling medical system experiencing various growing pains. Still, the legacy of the department would be long, as many medical professionals who joined the post-apartheid Department of Health in South Africa had been trained in exile during the liberation struggle. Reconstructing Medical Practice examines how doctors see health care and their place in it, why they remain in medicine and why they are limited in their ability to lead change in the current system. Doctors are beset by doubts and feel rejected by systems where they should be leaders - some see their role as 'flog[ging] a derelict system to get the last breath of workability out ... for their patients'. Others simply turn away. Rigorous studies carried out at large public teaching hospitals in Australia found that doctors were reluctant to increase safety in the wider health system, despite making every effort for their 'own' patients. Doctors' self-esteem was found to be delicate due to the uncertain nature of their work; colleagues provide the support doctors need to deliver good care. However, these essential relationships and their cherished connections with patients have disadvantages: reducing doctors' ability to admit to error. On top of this, senior doctors predict a future bereft of professional values - one where medicine is 'just a job'. While the loss of professional identity introduces new risks for patients and doctors, the repercussions of the more self-serving attitudes of younger doctors are unknown. Reconstructing Medical Practice concludes that regulation, despite its recent proliferation, is a clumsy and limited approach to ensuring good care. It presents original and much-needed ideas for ways to rebuild the critical relationship between doctors and the system. By better valuing communicative interactions and workplace relationships, safe and satisfying medical practice can be reconstructed.
