The book is provided open access under a CC BY 4.0 license. This book covers all aspects of minimally invasive glaucoma surgery (MIGS) and provides detailed information on each MIGS device, including its mechanism of action; patient selection; implantation techniques; post-operative management; and a review of the existing literature. Step-by-step descriptions are provided for the surgical technique used in implanting each MIGS device, accompanied by clear photographs of each surgical stage. Other areas covered include intra-operative gonioscopy (with tips on optimising the view of the anterior chamber angle) and the management of the intra-operative and post-operative complications. Essential information on the anatomy and physiology of the different aqueous outflow pathways is also included. A separate chapter addresses the introduction of MIGS globally, including the consideration of different reimbursement environments and the different types of glaucoma, e.g. angle closure glaucoma. This book will assist both glaucoma surgeons and general ophthalmologists in overcoming the learning curve involved in performed MIGS, by providing valuable and practical clinical pearls.

Current Developments in Glaucoma Surgery and MIGS by J.R. Samples,I.I.K. Ahmed Pdf

Successful interventional glaucoma remains a sought-after goal for the glaucoma clinician for many reasons. Medical costs, compliance, and adherence remain major barriers for the treatment of our patients. A truly definitive treatment for glaucoma is most likely to be largely surgical, as it is doubtful that patients will continue to use glaucoma drugs in the future for anything other than transient lowering of intraocular pressure. In the coming era, it seems probable that neuroprotection and neurorestoration will be combined with MIGS to lower pressure. The different types of MIGS are increasing in proportion to their efficacy. To date, there are no fewer than 12 choices available to glaucoma specialists. Choice of procedure depends on the underlying anatomy as well as the patient’s individual characteristics. Truthfully, the number of available surgeries depends upon their taxonomy. Most are based on the outflow system and the canal, as illustrated in this book.

Marijuana As Medicine? by Institute of Medicine,Janet Joy,Alison Mack Pdf

Some people suffer from chronic, debilitating disorders for which no conventional treatment brings relief. Can marijuana ease their symptoms? Would it be breaking the law to turn to marijuana as a medication? There are few sources of objective, scientifically sound advice for people in this situation. Most books about marijuana and medicine attempt to promote the views of advocates or opponents. To fill the gap between these extremes, authors Alison Mack and Janet Joy have extracted critical findings from a recent Institute of Medicine study on this important issue, interpreting them for a general audience. Marijuana As Medicine? provides patientsâ€"as well as the people who care for themâ€"with a foundation for making decisions about their own health care. This empowering volume examines several key points, including: Whether marijuana can relieve a variety of symptoms, including pain, muscle spasticity, nausea, and appetite loss. The dangers of smoking marijuana, as well as the effects of its active chemical components on the immune system and on psychological health. The potential use of marijuana-based medications on symptoms of AIDS, cancer, multiple sclerosis, and several other specific disorders, in comparison with existing treatments. Marijuana As Medicine? introduces readers to the active compounds in marijuana. These include the principal ingredient in Marinol, a legal medication. The authors also discuss the prospects for developing other drugs derived from marijuana's active ingredients. In addition to providing an up-to-date review of the science behind the medical marijuana debate, Mack and Joy also answer common questions about the legal status of marijuana, explaining the conflict between state and federal law regarding its medical use. Intended primarily as an aid to patients and caregivers, this book objectively presents critical information so that it can be used to make responsible health care decisions. Marijuana As Medicine? will also be a valuable resource for policymakers, health care providers, patient counselors, medical faculty and studentsâ€"in short, anyone who wants to learn more about this important issue.

Glaucoma Surgery by R.N. Weinreb,P. Ramulu,F. Topouzis,K.H. Park,K. Mansouri,S.F. Lerner Pdf

Glaucoma surgery was the topic of the eleventh World Glaucoma Association Consensus meeting. Glaucoma surgery in primary open-angle glaucoma was also the topic of the second Consensus meeting held in 2005. For the current version, angle closure and angle-closure glaucoma, as well as the secondary glaucomas, were also included. But the expansion of the subject matter is not the only reason for the timing of this new Consensus. Perhaps even more important is the explosion of new surgical ideas and techniques, which are revolutionizing the surgical approach to the glaucoma patient. The idea of targeting anatomic outflow and inflow pathways and individualizing care has opened so many new possibilities for improving surgical outcomes. Furthermore, outcomes are no longer only focused on lowering intraocular pressure, but also on enhancing health- and vision-related quality of life. As with prior meetings, it was a daunting task to seek and obtain consensus on a broad subject matter for which there are so many different opinions. As it is unclear how each of us decides how we practice and the evidence to guide us often is sparse, this consensus — as well as the others — is based not only on the published literature, but also on expert opinion. Although consensus does not replace and is not a surrogate for scientific investigation, it does provide considerable value, especially when the desired evidence is lacking. The goal of this consensus is to provide a foundation for glaucoma surgery and how it can be best done in clinical practice. Identification of those areas for which we have little evidence, and therefore the need for additional research, always is a high priority. We hope that this consensus report will serve as a benchmark of our understanding of what is known and what we would like to know. However, this Consensus Report is expected to be just a beginning that will be revised and improved with the emergence of new evidence, and new and improved surgical procedures.

Childhood Glaucoma by R.N. Weinreb,A.L. Grajewski,M. Papadopoulos,J. Grigg,S. Freedman Pdf

Preface Childhood is the topic of the ninth World Glaucoma Association Consensus. There has been only sparse attention to the diagnosis and treatment of childhood glaucoma. Both pediatric ophthalmologists and glaucoma specialists provide care for such children. In some instances, they manage these individuals alone and, in others, the management is shared. For this consensus, the participation of both groups was solicited. The global faculty, consisting of leading authorities on the clinical and scientific aspects of childhood glaucoma, met in Vancouver on July 16, 2013, just prior to the World Glaucoma Congress, to discuss the reports and refine the consensus statements. As with prior meetings, it was a daunting task to seek and obtain consensus on such a complicated and nuanced subject. It is unclear how each of us decides how we practice, and evidence to guide us often is sparse. It is remarkable how few high level studies have been conducted on the management of childhood glaucoma. Hence, this consensus, as with the others, is based not only on the published literature, but also on expert opinion. Although consensus does not replace and is not a surrogate for scientific investigation, it does provide considerable value, especially when the desired evidence is lacking. The goal of this consensus was to provide a foundation for diagnosing and treating childhood glaucoma and how it can be best done in clinical practice. Identification of those areas for which we have little evidence and, therefore, the need for additional research also was a high priority. We hope that this consensus report will serve as a benchmark of our understanding. However, this consensus report, as with each of the others, is intended to be just a beginning. It is expected that it will be revised and improved with the emergence of new evidence. Robert N. Weinreb, Chair

Glaucoma Surgery by P. Bettin,P.T. Khaw Pdf

The first edition of this volume, published in 2012, has become an indispensable reference for clinicians, a fact that inspired the making of this 2nd edition. The book starts with a discussion of current achievements and limitations of the medical therapy of glaucoma. The focus is then placed on conventional glaucoma procedures such as trabeculectomy, deep sclerectomy, and glaucoma drainage device surgery. A chapter is dedicated to the postoperative management of filtering procedures. Further chapters review the role of the ocular surface and the issue of modulation of the scarring processes in glaucoma surgery. The second section covers some promising new devices and techniques. The final part addresses the surgical approaches to some specific clinical challenges: angle-closure glaucoma, concomitant cataract and glaucoma, pediatric glaucoma, and refractory glaucomas. This volume provides residents and general ophthalmologists with valuable new insights into the fascinating and sometimes complex aspects of glaucoma surgery. It will also assist glaucoma specialists when they review and validate therapeutic approaches with their patients.
