In May 2002 Timor Leste (East Timor) emerged as a new nation after centuries of foreign rule and decades of struggle for independence. Its birth was a painful one; a United Nations-brokered Popular Consultation in August 1999, in which an overwhelming majority of the people opted for independence, was followed by several weeks of vengeful violence, looting, and destruction by pro-Indonesia militias. It left the territory and all of its essential services devastated. In this context, the United Nations Transitional Administration in East Timor (UNTAET), with the country's leaders and people and many other partners, set about restoring order and services, building a government structure, and preparing for independence. This paper summarizes the rehabilitation and development of the health sector from early 2000 to the end of 2001.

Development assistance to fragile states and conflict-affected areas can be a core component of peacebuilding, providing support for the restoration of government functions, delivery of basic services, the rule of law, and economic revitalization. What has worked, why it has worked, and what is scalable and transferable, are key questions for both development practice and research into how peace is built and the interactive role of domestic and international processes therein. Despite a wealth of research into these questions, significant gaps remain. This volume speaks to these gaps through new analysis of a selected set of well-regarded aid interventions. Drawing on diverse scholarly and policy expertise, eight case study chapters span multiple domains and regions to analyse Afghanistan’s National Solidarity Programme, the Yemen Social Fund for Development, public financial management reform in Sierra Leone, Finn Church Aid’s assistance in Somalia, Liberia’s gender-sensitive police reform, the judicial facilitators programme in Nicaragua, UNICEF’s education projects in Somalia, and World Bank health projects in Timor-Leste. Analysis illustrates the significance of three broad factors in understanding why some aid interventions work better than others: the area of intervention and related degree of engagement with state institutions; local contextual factors such as windows of opportunity and the degree of local support; and programme design and management. Building and operating successful public institutions is a perennial and long-term challenge for governments, which is compounded by the volatile conditions found in fragile settings. Yet some government agencies do manage to take root and achieve success in delivering results earning legitimacy and forging resilience in otherwise challenging contexts. Drawing on mixed-method empirical research carried out on nine public agencies in Lao PDR, Sierra Leone, The Gambia, and Timor Leste, this volume identifies the shared causal mechanisms underpinning institutional success in fragile states by examining the inner workings of these institutions, along with the external operational environment and sociopolitical context in which they exist. Successful institutions share and deploy a common repertoire of internal and external operational strategies. In addition they connect this micro-institutional repertoire to the macro-sociopolitical context along three discernible pathways to institutional success. Institutional development is a heavily contextual, dynamic, and non-linear process but certain actionable lessons emerge for policy-makiers and development partners.

Research in recent years on aid effectiveness shows that significant obstacles in fragile states - insecurity, poor governance and weak implementation capacity - usually prevent aid from achieving the desired results in these environments. This study investigates the attributes and effectiveness of donor-supported programmes and projects that worked well under difficult conditions in fragile states. Presented in this study are nine development initiatives in six less developed countries - Afghanistan, Cambodia, Mozambique, Sierra Leone, Timor Leste and Uganda. The cases show that development initiatives, which engage local communities and local level governments, are often able to have significant impact. However, for more substantial improvements to take places, localized gains need to be scaled up either horizontally (other localities) or vertically (to higher levels). Given the advantages of working at the local level and the difficulty of working through mainstream bureaucratic agencies at higher levels in these countries, donors often prefer to create 'parallel-agencies' to reach out to larger numbers of beneficiaries. However, this may in the long run weaken the legitimacy of mainstream government institutions, and donor agencies may therefore choose to work as closely as possible with government officials from the beginning to build trust and demonstrating that new initiatives are non-threatening and help prepare the eventual mainstreaming of 'parallel agencies'.

Providing medical support to the local population during a chronic crisis is difficult. The crisis in the Democratic Republic of the Congo (DRC), which is characterized by high excess mortality, ongoing armed violence, mass forced displacement, interference by neighboring countries, resource exploitation, asset stripping, and the virtual absence of the state, has led to great poverty and a dearth of funds for the support of the health system. International nongovernmental organizations (NGOs) have stepped in to address the dire humanitarian situation. This study looks at four organizations that support local health care in the eastern DRC: the International Rescue Committee (IRC), Malteser, Medical Emergency Relief International (Merlin), and the Association Régionale d'Approvisionnement en Médicaments Essentiels (ASRAMES). The study makes a comparison of the management and financing approaches of these four organizations by collecting and comparing qualitative and quantitative data on their interaction with the (remaining) local health providers and the local population. Specific objectives of the study are: 1. To identify which management and financing approaches, including the setting of fees, are used by the four NGOs supporting healthcare in the eastern DRC. 2. To determine how these financing approaches affect utilization rates in the health zones supported by the four NGOs. 3. To assess how these utilization rates compare with donor and humanitarian standards. 4. To determine at what level fees must be set to allow for cost recovery or cost sharing in health facilities. 5. To identify the managerial problems confronting the four NGOs. Many epidemiological and public health studies focus on the interaction between health providers and target groups. Supporting Local Health Care in a Chronic Crisis: Management and Financing Approaches in the Eastern Democratic Republic of the Congo concentrates more on how the relationship between the supporting NGOs and the local health system actually develops. In addition, a common aspect of many of the epidemiological and public health studies is the search for an optimal, or at least appropriate, management and financing approach.
