Surgical site infections are caused by bacteria that get in through incisions made during surgery. They threaten the lives of millions of patients each year and contribute to the spread of antibiotic resistance. In low- and middle-income countries, 11% of patients who undergo surgery are infected in the process. In Africa, up to 20% of women who have a caesarean section contract a wound infection, compromising their own health and their ability to care for their babies. But surgical site infections are not just a problem for poor countries. In the United States, they contribute to patients spending more than 400 000 extra days in hospital at a cost of an additional US $10 billion per year. No international evidence-based guidelines had previously been available before WHO launched its global guidelines on the prevention of surgical site infection on 3 November 2016, and there are inconsistencies in the interpretation of evidence and recommendations in existing national guidelines. These new WHO guidelines are valid for any country and suitable to local adaptations, and take account of the strength of available scientific evidence, the cost and resource implications, and patient values and preferences.

Fractured rock is the host or foundation for innumerable engineered structures related to energy, water, waste, and transportation. Characterizing, modeling, and monitoring fractured rock sites is critical to the functioning of those infrastructure, as well as to optimizing resource recovery and contaminant management. Characterization, Modeling, Monitoring, and Remediation of Fractured Rock examines the state of practice and state of art in the characterization of fractured rock and the chemical and biological processes related to subsurface contaminant fate and transport. This report examines new developments, knowledge, and approaches to engineering at fractured rock sites since the publication of the 1996 National Research Council report Rock Fractures and Fluid Flow: Contemporary Understanding and Fluid Flow. Fundamental understanding of the physical nature of fractured rock has changed little since 1996, but many new characterization tools have been developed, and there is now greater appreciation for the importance of chemical and biological processes that can occur in the fractured rock environment. The findings of Characterization, Modeling, Monitoring, and Remediation of Fractured Rock can be applied to all types of engineered infrastructure, but especially to engineered repositories for buried or stored waste and to fractured rock sites that have been contaminated as a result of past disposal or other practices. The recommendations of this report are intended to help the practitioner, researcher, and decision maker take a more interdisciplinary approach to engineering in the fractured rock environment. This report describes how existing tools-some only recently developed-can be used to increase the accuracy and reliability of engineering design and management given the interacting forces of nature. With an interdisciplinary approach, it is possible to conceptualize and model the fractured rock environment with acceptable levels of uncertainty and reliability, and to design systems that maximize remediation and long-term performance. Better scientific understanding could inform regulations, policies, and implementation guidelines related to infrastructure development and operations. The recommendations for research and applications to enhance practice of this book make it a valuable resource for students and practitioners in this field.

Qualifying as an Army research psychologist requires, first of all, a Ph.D. from a research (not clinical) intensive graduate psychology program. Due to their advanced education, research psychologists receive a direct commission as Army officers in the Medical Service Corps at the rank of captain. In terms of numbers, the 71F AOC is a small one, with only 25 to 30 officers serving in any given year. However, the 71F impact is much bigger than this small cadre suggests. Army research psychologists apply their extensive training and expertise in the science of psychology and social behavior toward understanding, preserving, and enhancing the health, well being, morale, and performance of Soldiers and military families. As is clear throughout the pages of this book, they do this in many ways and in many areas, but always with a scientific approach. This is the 71F advantage: applying the science of psychology to understand the human dimension, and developing programs, policies, and products to benefit the person in military operations. This book grew out of the April 2008 biennial conference of U.S. Army Research Psychologists, held in Bethesda, Maryland. This meeting was to be my last as Consultant to the Surgeon General for Research Psychology, and I thought it would be a good idea to publish proceedings, which had not been done before. As Consultant, I'd often wished for such a document to help explain to people what it is that Army Research Psychologists "do for a living." In addition to our core group of 71Fs, at the Bethesda 2008 meeting we had several brand-new members, and a number of distinguished retirees, the "grey-beards" of the 71F clan. Together with longtime 71F colleagues Ross Pastel and Mark Vaitkus, I also saw an unusual opportunity to capture some of the history of the Army Research Psychology specialty while providing a representative sample of current 71F research and activities.
