The organization was in the hands of Klaus Assmann and Fritz Bocker In the last decade medical imaging has undergone a rapid development New imaging modalities such as Computer Tomography (CT), Digital Angiography (DSA) and Magnetic Resonance Imaging (MRI) were developed using the capabilities of modern computers. In a modern hospital these technologies produce already more then 25% of image data in digital form. This format lends itself to the design of computer assisted Information systems Integrating data acquisition, presentation, communi cation and archiving for all modalities and users within a department or even a hospital. Advantages such as rapid access to any archived Image, synoptic presentation, computer assisted image analysis to name only a few, are expected. The design of such pictorial information systems, however, often called PACS (Picture Archiving and Communication Systems) In the medical community is a non-trivial task involving know-how from many disciplines such as - Medicine (especially Radiology), - Data Base Technology, - Computer Graphics, - Man Machine Interaction, - Hardware Technology and others. Most of these disCiplines are represented by disjunct scientific communities.

These proceedings are the accumulation of eight years of research and development results in PACS by various dedicated groups throughout the world. The purpose of this institute was to review the most recent technology available for PACS and some clinical results. The readers should notice the remarkable advances in this field by comparing the contents in these proceedings with those in a previous institute on "Pictorial Information Systems in Medicine" held August 27 - September 7, 1984 in Braunlage/Harz, Federal Republic of Germany, and published as Vol. 19 in this series. The institute was organized according to four categories: PACS components and system integration, PACS and related research in various countries and manufacturing companies, clinical experience and research support, and participants' scientific communications. The 33rd Annual Meeting of the German Association for Medical Documentation, Informatics and Statistics was combined with a Special Topic Conference of the European Federation for Medical Informatics and takes place at Hannover, F. To commemorate his contribution to the development of medicine the conference was devoted to him "Peter Reichertz Memorial Conference on Expert Systems and Decision Support in Medicine" Since computers in the early Fifties were first applied to support medical reasoning, various phases of euphoria and resi~ation have . followed. Every new methodology which became technically possible was and will be applied to the old questlon of how to diagnose diseases more reliably. Artificial Intelligence is just one new approach to the old challenge. Over the years some· authors have been very optimistic and put forward opinions which motivated the common press to coin the phrase 'Dr. med. computer'. Papers printed under this heading rebuffed the majority of physiCians for many years. Today we know that medical decision making is a most complex buman performance. And 30 years of research on decision support have given us only limited insight into the underlying processes. Most of the principal methodological questions were already asked very early on.

As its name implies, this book deals with clinical information systems. The clinical information system (or CIS) is an automated system with a long term database containing clinical information used for patient care. This definition excludes business systems (no clinical data), physiological monitoring systems (no long term database), and many research systems (not used in patient care). The theses of this book are (a) that CIS technology is mature, (b) that the CIS will have a major impact upon patient care and the health delivery system, and (c) that the number of commercial systems which now offer these potential benefits is very small. The objective of this book is to establish the above theses and thereby (a) inform both users and developers, (b) increase the demand for more sophisticated products, and finally, (c) provide marketplace incentives to advance the state of the art. The CIS is an application of computer technology for a specific class of problems. Its development requires a knowledge of the technology with an understanding of the application area. As with any tool-based application, the scope of the product will be limited by the capability of the tool. In the case of the CIS, reliable computers with comprehensive database facilities became commercially available in the early 1970s. By the mid 1970s there was a maturation of the literature, and evaluations of 5-years' use began to appear. As will be shown, there have been surprisingly few new ideas introduced since the 1970s.

Although system analysis is a well established methodology, the specific application of such analysis to information systems is a relatively new endeavor.
