This book uses the story of one of the authors, Gus White, as a way to talk about unconscious biases and their consequences to the medical profession and beyond. White is an orthopedic surgeon, who grew up in Tennessee under Jim Crow, went to Brown, and was the only black student at Stanford Medical School. He was the first black chief resident at Yale, the only black surgeon in Vietnam, and was the first black chief of service in a Harvard teaching hospital. His life spans an enormous change in American race relations, and he has many eye opening stories to tell. His description of his early years in an extremely segregated and racist society now reads like something from another world. White and Chanoff want to use the autobiographical approach of this book to show how great the disparities still are, and make the case for “culturally competent” medical training, in a way that is more vivid and memorable than a research review or policy paper. The book looks at White’s life, but always with an eye to what moved him to the idea of equality in medicine and problems of disparities in medicine.

Being a patient is a unique interpersonal experience but it is also a universal human experience. The relationships formed when we are patients can also teach some of life's most important lessons, and these relationships provide a special window into ethics, especially the ethics of healthcare professionals. This book answers two basic questions: As patients see it, what things allow relationships with healthcare providers to become therapeutic? What can this teach us about healthcare ethics? This volume presents detailed descriptions and analyses of 50 interviews with 58 patients, representing a wide spectrum of illnesses and clinician specialties. The authors argue that the structure, rhythm, and horizon of routine patient care are ultimately grounded in patient vulnerability and clinician responsiveness. From the short interview segments, the longer vignettes and the full patient stories presented here emerge the neglected dimensions of healthcare and healthcare ethics. What becomes visible is an ethics of everyday interdependence, with mutual responsibilities that follow from this moral symbiosis. Both professional expressions of healthcare ethics and the field of bioethics need to be informed and reformed by this distinctive, more patient-centered, turn in how we understand both patient care as a whole and the ethics of care more specifically. It is the young women of Sylhet who are most affected. The global economy has increased competition for resources and led to marriage being seen as a route to economic advancement. Parents prefer to give their daughters in marriage to families that will widen their social contacts and enhance their economic and social standing. Accordingly, the young wife's outsider status (and hence vulnerability to mental illness) has increased as it is no longer customary to give daughters in marriage to local kin. Yet, patients and their families do not work out tensions passively. They are active agents in the construction of their own diagnosis. The extent to which patients act or are acted upon is an investigation that runs throughout the book.
