The Identification and Treatment of Gait Problems in Cerebral Palsy by James R. Gage,Michael H. Schwartz,Steven E. Koop,Tom F. Novacheck Pdf

The only book to deal specifically with the treatment of gait problems in cerebral palsy, this comprehensive, multi-disciplinary volume will be invaluable for all those working in the field of cerebral palsy and gait (neurologists, therapists, physiatrists, orthopaedic and neurosurgeons, and bioengineers). The book is divided into two parts. The first is designed to help the reader evaluate and understand a child with cerebral palsy. It deals with neurological control, musculoskeletal growth, and normal gait, as well as cerebral injury, growth deformities and gait pathology in children with cerebral palsy. The second section is a comprehensive overview of management. It emphasizes the most fundamental concept of treatment: manage the child's neurologic dysfunction first and then address the skeletal and muscular consequences of that dysfunction. The book has been thoroughly updated since the previous edition, with a greater focus on treatment and several entirely new topics covered, including chapters on the operative treatment of orthopaedic deformities. Computerized Gait Analysis in the Rehabilitation of Children with Cerebral Palsy and Spina Bifida by Jo-Anne Tomie,Alberta Heritage Foundation for Medical Research,Hailey, David Pdf

The Glenrose Hospital, Edmonton is considering routine clinical use of computerized gait analysis techniques in the management of children with cerebral palsy or spina bifida who have significant walking disorders. About 100 children per year could be examined to assist with decisions on their treatment. Evidence of clinical benefit from use of modern gait analysis techniques is sparse. These technologies seem helpful in detecting within-subject gait changes, and between-subject gait differences. However, available evidence is insufficient to draw conclusions about the effects of computerized gait analysis on treatment outcomes. Cost per examination with a facility of the sort proposed by the hospital might be of order of $1,800 - $2,200. Cost per patient would depend on the extent of follow-up gait analysis after treatment. On the basis of information available for this report, computerized gait analysis is a potentially useful technology in the clinical management of persons with walking disabilities, but its efficacy is not established. It is suggested that, if computerized gait analysys is adopted for routine clinical use by the Glenrose hospital, it should be regarded as a developing technology and its application linked to systematic collection and assessment of patient outcomes and cost data under well-defined protocols.

Cerebral Palsy by Freeman Miller,Steven J. Bachrach Pdf

When a child has a health problem, parents want answers. But when a child has cerebral palsy, the answers don't come quickly. A diagnosis of this complex group of chronic conditions affecting movement and coordination is difficult to make and is typically delayed until the child is eighteen months old. Although the condition may be mild or severe, even general predictions about long-term prognosis seldom come before the child's second birthday. Written by a team of experts associated with the Cerebral Palsy Program at the Alfred I. duPont Hospital for Children, this authoritative resource provides parents and families with vital information that can help them cope with uncertainty. Thoroughly updated and revised to incorporate the latest medical advances, the second edition is a comprehensive guide to cerebral palsy. The book is organized into three parts. In the first, the authors describe specific patterns of involvement (hemiplegia, diplegia, quadriplegia), explain the medical and psychosocial implications of these conditions, and tell parents how to be effective advocates for their child. In the second part, the authors provide a wealth of practical advice about caregiving from nutrition to mobility. Part three features an extensive alphabetically arranged encyclopedia that defines and describes medical terms and diagnoses, medical and surgical procedures, and orthopedic and other assistive devices. Also included are lists of resources and recommended reading.

Understanding in Children with Cerebral Palsy: Orthopedic Problems by Dmitry Popkov Pdf

Cerebral palsy presents stable encephalopathy that systemically influences on the child's and adult's organism. Orthopedic complications of cerebral palsy possess on the contrary progressive features and those complications cause gradual failure of motor ability of a person: impaired walking, loss of postural ability, reduced self-care and inclusion, finally quality of life becoming much worse. That is why orthopedic aspects are of great importance in children with cerebral palsy. This volume presents a collection of chapters covering a wide range of problems, from a system of universal classification of dysfunctions in Cerebral Palsy and practical issues of prevention of orthopedic complications orthopedic to surgery (diagnostics and quantitative interpretation of gait disorders in such patients, optimal methods of surgical intervention, hip surgery, spinal surgery) and aspects of nutritive status of this specific category of patients, associated comorbidities and their influence and connection with orthopedic problems. Furthermore, three chapters are dedicated to the anesthetic aspects of orthopedic surgery. The substantiation is given to the methods of combined anesthesia and sedation during surgery, as well as in the immediate postoperative period, taking into account concomitant epilepsy, increased intracranial pressure and spasticity in such children.Single-event multilevel orthopedic surgery is a recognized gold standard in ambulatory patients, though it varies according to gait patterns and associated orthopedic problems causing motor disorders. The book contains explanations and methodology of surgeries for conventional gait patterns, as well as in iatrogenic disorders caused by previous unreasonable interventions. The chapter on the correction of deformities and dysfunctions of the upper limb shows the first applied transfer of m.flexor cubitus ulnaris to the radius, which gave extremely positive results both in terms of function and aesthetics.Unfortunately, in Russia the number of CP patients with iatrogenic orthopedic disorders is high. Extremely negative consequences in the form of weakness of the triceps muscles after percutaneous fibromyotomies (so-called "Ulzibat method") or lengthening of the Achilles tendon in diplegia performed before the age of five years and before the development of muscle retraction lead to the development of crouch gait even in patients with high motor potential (GMFCS I, II). This complication of unreasonable surgical interventions develops in 5-7 years after primary surgery and in the opinion of doctors and parents is not related to previous procedures. This book, based on 3D-instrumented gait analysis, shows the features of iatrogenic crouch gait, as well as methods of surgical correction of this difficult situation. The data presented in this book are based, on the one hand, on the personal practical and scientific experience of each author, and on the other hand, the entire team of authors works in collaboration with each other, coordinating their decisions and interventions with colleagues, thus ensuring continuity in treatment of children and adults suffering from cerebral palsy.

Handbook of Human Motion by Anonim Pdf

The Handbook of Human Motion is a large cross-disciplinary reference work which covers the many interlinked facets of the science and technology of human motion and its measurement. Individual chapters cover fundamental principles and technological developments, the state-of-the-art and consider applications across four broad and interconnected fields; medicine, sport, forensics and animation. The huge strides in technological advancement made over the past century make it possible to measure motion with unprecedented precision, but also lead to new challenges. This work introduces the many different approaches and systems used in motion capture, including IR and ultrasound, mechanical systems and video, plus some emerging techniques. The large variety of techniques used for the study of motion science in medicine can make analysis a complicated process, but extremely effective for the treatment of the patient when well utilised. The handbook descri bes how motion capture techniques are applied in medicine, and shows how the resulting analysis can help in diagnosis and treatment. A closely related field, sports science involves a combination of in-depth medical knowledge and detailed understanding of performance and training techniques, and motion capture can play an extremely important role in linking these disciplines. The handbook considers which technologies are most appropriate in specific circumstances, how they are applied and how this can help prevent injury and improve sporting performance. The application of motion capture in forensic science and security is reviewed, with chapters dedicated to specific areas including employment law, injury analysis, criminal activity and motion/facial recognition. And in the final area of application, the book describes how novel motion capture techniques have been designed specifically to aid the creation of increasingly realistic animation within films and v ideo games, with Lord of the Rings and Avatar just two examples. Chapters will provide an overview of the bespoke motion capture techniques developed for animation, how these have influenced advances in film and game design, and the links to behavioural studies, both in humans and in robotics. Comprising a cross-referenced compendium of different techniques and applications across a broad field, the Handbook of Human Motion provides the reader with a detailed reference and simultaneously a source of inspiration for future work. The book will be of use to students, researchers, engineers and others working in any field relevant to human motion capture.

Foot and Ankle Motion Analysis by Gerald F. Harris,Peter A. Smith Pdf

Human motion analysis or gait analysis is used throughout the country and the world in clinics for pre-surgical planning and postsurgical follow-up. Only recently have technological advances truly begun to meet medical needs by supplying more accurate analytical data from which to make educated assessments of dynamic foot and ankle pathology. A comprehensive overview of current and emerging methods is necessary for practitioners to effectively integrate the new techniques into better pre-treatment planning, surgical and rehabilitative care, and post-treatment follow-up. Originating as a one-day workshop sponsored by the Shriner’s Hospitals and the National Institutes of Health, Foot and Ankle Motion Analysis: Clinical Treatment and Technology provides a single source reference for the latest technologies and their clinical applications. With contributions from an international panel of experts from orthopaedic, rehabilitation, engineering, academic, medical-industrial, and clinical disciplines, this text focuses on the relevant scientific advances with an emphasis on applications, limitations, and problems to be solved. Divided into two parts, the text begins by presenting basic and advanced clinical applications and opportunities in foot and ankle motion analysis in both pediatric and adult cases. The second part introduces the technological advances themselves from a quantitative perspective. Modeling concepts, seminal developments, and novel approaches are described along with emerging horizons related to mechanical paradigms, imaging, kinetics, robotics and simulation, tri-planar force sensing, and more. The book also includes a chapter of references and sources of support for future research and development prospects. Clinical and research applications in motion analysis have resulted in better functional assessment, fewer, more effective surgeries, and longer-term follow-up care. Foot and Ankle Motion Analysis: Clinical Treatment and Technology provides a basis for expanding these contributions to the broader community of practitioners caring for both adult and pediatric patients.

Analysis of Lower Limb Skeletal Geometry in Children with Cerebral Palsy [microform] by Kee Tang Pdf

Clinical gait assessment of children with cerebral palsy (CP) is common practise, yet the gait models used are often based on the anthropometry of average-sized healthy adults. This study investigated the variability in skeletal landmarks of the lower extremities between able-bodied adults, able-bodied children, and children with spastic CP. The accuracy of femoral anteversion (FAV) and tibial torsion (TT) measurements from clinical and MRI estimates was examined. Statistical analysis on landmark data showed very little significant differences between the two child groups, however, a substantially higher number of significant differences were found between the able-bodied adults and both child groups. Clinical measurements of FAV and TT were found to be poor predictors of the MRI measurements. FAV and TT were sensitive to the estimate of the shaft axis. Findings from this study provide a strong motivation for developing better scaling and landmark prediction methods for children with CP.

Cerebral Palsy by Christos P. Panteliadis Pdf

This third edition systematically reviews recent developments in the diagnosis and evidence-based treatment of cerebral palsy, a consequence of foetal and early infant brain damage resulting in lifelong disabilities with a range of clinical characteristics. The first part discusses the definition, aetiology, classification, imaging and neuropathology, while the second focuses on the management of the individual challenges that children with cerebral palsy face, such as spasticity, dyskinesia, feeding problems and scoliosis. Based on the diverse characteristics of cerebral palsy, children require care from various specialists, including neuro-paediatricians, orthopaedists, psychologists, epidemiologists, physiotherapists and occupational therapists. This work was written by an international team of such specialists, providing a comprehensive mix of perspectives and expertise.
