Excerpt from The American Journal of Obstetrics and Diseases of Women and Children, Vol. 51: January-June, 1905 No special introducing forceps or syringe are necessary, al though some have been described. I use, however, nearly always a forceps with a pelvic curve and fenestrated blades for the greater ease in the introduction, yet a sponge holder will do. The syringe employed is an ordinary Davidson's. The glass varieties are very apt to break. The metal ones get out of order and can not be boiled. Of course, the asepsis must be perfect in every detail. It is well to insert the largest size bag possible at the outset and to remember in filling the balloon with the fluid to inflate slowly with slight traction on the tube so that the head, if presenting, will be displaced as little as possible. The patient is left to nature's efforts after the introduction of the balloon unless there is need of hurry when traction can be made at regular intervals on the tube, increasing thereby the strength of the pains and accelerating the dilatation of the cervix. It is only necessary to insert successive bags in cases where great haste ls demanded and where the pains are few and far between or stop altogether after the first bag is expelled. Results - As before stated, in a previous article, the writer re ported the first series of cases in which the balloons were used in the service at the Sloane Maternity Hospital. These were 72 in number out of deliveries for the two years up to September 1, 1899. The cases selected had sure indications for interfering, and were those in which other methods had been tried and had failed. In all advantages were obtained and the field of useful ness of the balloons was more definitely determined. This second series I want to report so as to further emphasize the utility of the bags and the great amount of dependence we place upon them, especially in private practice. These cases also are not selected but I publish all in which bal loons were employed, the first set occurring in the first 200 cases of my private practice; the second set occurring in the four years ending September 1, 1903, in deliveries, subsequent to those before reported, from the service of the Sloane Maternity Hospital. Oper ated on January 5, 1900, for extrauterine pregnancy. She re turned ou April and was operated on April 26, when a tubo-ovarian cyst on the left side was removed. She complained, on the second occasion, of pains in the abdomen, chiefly on the left side. Had missed one week, but had no uterine hemorrhage. The mass could be felt on physical examination. Excerpt from The American Journal of Obstetrics and Diseases of Women and Children, Vol. 50: July-December, 1904 Since this case, I have made it a rule to examine the appendix whenever I was not completely satisfied that the patient's symp toms could be explained by the existing pelvic lesions. Like many another operator, however, I failed to record accurately my observations until some two years ago. Then with the View of throwing some light upon the question considered above I began systematically to remove the appendix in every case where the abdomen was opened for other purposes, unless, in my opinion, the patient's chances of recovery were jeopardized by the addi tional five minutes' time required for the removal. Each appendix thus removed was subjected to careful microscopic examination by Dr. R. L. Morse, pathologist of the gynecologic service of the Hospital of the University of Michigan. His work in turn was supervised by and his results verified in each case by Professor A. S. Warthin, director Of the Pathologic Laboratories of the University of Michigan. If any great amount of good is to be accomplished the decision must he arrived at and the hysterectomy performed early - the earlier the better the success. In attempting to arrive at an early decision there will be the greater danger of Operating upon patients who would otherwise have recovered without this interference. The greatest care and discrimination will consequently be necessary in deciding for or against the radical procedure and the more skilled the physician in diagnosis the fewer mistakes he will make pro or con. After all has been said and done the distinction between the different varieties is clinically not easy and, as a matter of fact, the cases are generally of a mixed nature.
