The Operational Evidence Base for Delivering Direct Nutrition Interventions in India by Rasmi Avula,Suneetha Kadiyala,Kavita Singh,Purnima Menon Pdf

The persistence of undernutrition in the face of India’s impressive economic growth is of enormous concern. Less than 55 percent of mothers and children receive any essential health and nutrition inputs that are critical for improving maternal and child nutrition. We conducted a desk review (1) to document the extent to which national and civil society/NGO programs in India reflect current technical recommendations for nutrition and (2) assess the operational evidence base for implementing essential interventions for nutrition in the Indian context. We reviewed the design of the two major national programs, Integrated Child Development Services (ICDS) and the National Rural Health Mission (NRHM). Subsequently, we used Google Scholar to search the published literature from 2000 to 2012 for evidence of interventions addressing the inputs to improve child nutrition. Finally, we contacted 70 program stakeholders to identify the unpublished evidence on inputs in program models implemented by civil society/nongovernment organizations. We find that, by design, the two national programs (ICDS and NRHM) together appear to incorporate all the essential inputs and use evidence-based interventions. There is an expectation by design that the frontline workers of ICDS and NRHM coordinate and collaborate to deliver the interventions. A review of 22 program models shows that a majority focused on improving breastfeeding and timely initiation of complementary feeding. However, only a few addressed the full spectrum of complementary feeding, vitamin A deficiency, pediatric anemia, and severe acute malnutrition. None addressed how to reduce intestinal parasitic burdens or prevent malaria. There is limited published literature on the effectiveness of the recommended interventions to deliver the essential inputs. There are few efficacy studies and even fewer effectiveness studies or program evaluations on delivering essential nutrition interventions in the Indian context. The most commonly used delivery strategies across multiple essential inputs were home visits that involved individual or group counseling by community health workers or by self-help groups. Mass media and community events such as marriages and fairs were used as avenues to generate support for the interventions. Some programs used community mobilization to promote the interventions. Several of these programs worked to improve coordination and convergence between ICDS and NRHM and to strengthen these existing systems through training, improved monitoring, and supervision. Overall, a large gap persists in both the published and gray literature on how to promote interventions to address the essential inputs. Much more operational evidence is needed to ensure high-quality delivery of the evidence-based interventions that are already being implemented nationwide. Given the potential for the national programs to effectively deliver interventions to achieve maximum coverage and impact, and the government of India’s current interest in ICDS system strengthening, this is an opportune time to test some of the innovations using the ICDS and NRHM platforms.

Learning together: Experimental evidence on the impact of group-based nutrition interventions in rural Bihar by Raghunathan, Kalyani,Kumar, Neha,Gupta, Shivani,Chauhan, Tarana,Kathuria, Ashi Kohli,Menon, Purnima Pdf

Despite improvements over the last decade or more, India still accounts for a large proportion of the global prevalence of maternal and child undernutrition. We use a cluster-randomized controlled design and two waves of panel data on more than 2000 households from Bihar to analyse the impact on diet quality and anthropometry of a health and nutrition intervention delivered through an at-scale women’s self-help group (SHGs) platform. We find that the intervention had small but significant impacts on women and children’s dietary diversity, with the main impacts coming from an increase in the consumption of fruits and vegetables and dairy, however, it had no impact on women’s body mass index. We identify several potential pathways to impact. To the extent that SHGs can effect broad-based social change, their current reach to millions of women makes them a powerful platform for accelerating improvements in maternal and child health and nutrition outcomes.

Tracking India’s progress on addressing malnutrition and enhancing the use of data to improve programs by Menon, Purnima,Avula, Rasmi,Sarswat, Esha,Mani, Sneha,Jangid, Manita Pdf

Data systems and their usage are of great significance in the process of tracking malnutrition and improving programs. The key elements of a data system for nutrition include (1) data sources such as survey and administrative data and implementation research, (2) systems and processes for data use, and (3) data stewardship across a data value chain. The nutrition data value chain includes the prioritization of indicators, data collection, curation, analysis, and translation to policy and program recommendations and evidence based decisions. Finding the right fit for nutrition information systems is important and must include neither too little nor too much data; finding the data system that is the right fit for multiple decision makers is a big challenge. Developed together with NITI Aayog, this document covers issues that need to be considered in the strengthening of efforts to improve the availability and use of data generated through the work of POSHAN Abhiyaan, India’s National Nutrition Mission. The paper provides guidance for national-, state-, and district-level government officials and stakeholders regarding the use of data to track progress on nutrition interventions, immediate and underlying determinants, and outcomes. It examines the availability of data across a range of interventions in the POSHAN Abhiyaan framework, including population-based surveys and administrative data systems; it then makes recommendations for the improvement of data availability and use. To improve monitoring and data use, this document focuses on three questions: what types of indicators should be used; what types of data sources can be used; and with what frequency should progress on different indicator domains be assessed.

Organizational partnerships for food policy research impact by Mabiso, Athur,Van Rheenen, Teunis,Ferguson, Jenna Pdf

Recently discourse has grown about the importance of partnerships for adding value to agriculture research, strengthening policy capacities, and enhancing food policy impact on global food security and poverty reduction. However, the literature on partnerships specifically focusing on food policy research impact is still mostly emerging. This paper contributes to our understanding of food policy–research partnerships and provides a review of the theory and empirical literature about the factors that contribute to effective food policy–research partnerships. The literature points to the emergence of organizational partnerships as primarily driven by subjective perceptions about potential partners, the complex and uncertain external environment, access to resources through partnership and expectations of potential impact of the partnership. Perceptions that are found to be important include trust, mutuality of partner goals, legitimacy of partner and the perception that partnering will achieve more than what one organization can accomplish independently through cost sharing, lower transactions costs and increased competitiveness. Effectively implementing each phase of the partnership cycle, from scoping to reviewing and revising, with effective communication throughout all phases, is critical to achieve high quality and impactful food policy–research partnerships.

2015 Global Hunger Index by von Grebmer, Klaus,Bernstein, Jill,de Waal, Alex,Prasai, Nilam,Yin, Sandra,Yohannes, Yisehac Pdf

The developing world has made progress in reducing hunger since 2000. The 2015 Global Hunger Index (GHI) shows that the level of hunger in developing countries as a group has fallen by 27 percent. Yet the state of hunger in the world remains serious. This marks the tenth year that IFPRI has assessed global hunger using this multidimensional measure. This report’s GHI scores are based on a new, improved formula that replaces the child underweight indicator of previous years with child stunting and child wasting. This change reflects the latest thinking on the most suitable indicators for child undernutrition, one of three dimensions of hunger reflected in the GHI formula. Across regions and countries, GHI scores vary considerably. Regionally, the highest GHI scores, and therefore the highest hunger levels, are still found in Africa south of the Sahara and South Asia. Despite achieving the largest absolute improvements since 2000, these two regions still suffer from serious levels of hunger. Levels of hunger are alarming or serious in 52 countries. Most of the eight countries with alarming GHI scores are in Africa south of the Sahara. While no countries are classified in the extremely alarming category this year, this high level of hunger could still exist. Due to insufficient data, 2015 GHI scores could not be calculated for places that recently suffered from high levels of hunger, including Burundi, Comoros, Democratic Republic of the Congo, Eritrea, Somalia, South Sudan, and Sudan. 2015 Global Hunger Index Interactive App: http://ghi.ifpri.org

Exploring strategic priorities for regional agricultural research and development investments in southern Africa by Johnson, Michael E.,Benin, Samuel,You, Liangzhi,Diao, Xinshen,Chilonda, Pius,Kennedy, Adam Pdf

An in-depth quantitative analysis is undertaken in this paper to assist the Southern African Development Community (SADC) Secretariat, member countries, and development partners in setting future regional investment priorities for agricultural research and development in the SADC region. A primary goal of this work was to identify a range of agricultural research priorities for achieving sector productivity and overall economic growth in southern Africa, at both the country and regional levels. This is accomplished by adopting an integrated modeling framework that combines a disaggregated spatial analytical model with an economywide multimarket model developed specifically for the region.

The Impact of Bolsa Família on Schooling by de Brauw, Alan,Gilligan, Daniel O.,Hoddinott, John F.,Roy, Shalini Pdf

We estimate the impact of Bolsa Família on a range of education outcomes, including school participation, grade progression, grade repetition, and dropout rates. Using a large-sample household panel survey from 2005–2009 collected for this evaluation, we develop a statistically balanced comparison group of eligible nonparticipant households and estimate impacts using propensity-score-weighted regression. We estimate that Bolsa Família increased average school participation among all children age 6 to 17 years by (a weakly significant) 4.5 percent. It had no effect on grade promotion, on average. However, within the subsample of girls, Bolsa Família caused substantial improvements in schooling outcomes, including significant increases in school participation (8.2 percent) and rates of grade progression (10.4 percent). We show that the gains in girls’ schooling do not derive from catch-up effects, but rather increase girls’ existing advantage in schooling attainment. In general, impacts are larger among older children, in rural areas, and in the Northeast.

Persisting Undernutrition in India by Nira Ramachandran Pdf

The book revisits the causes of persisting under nutrition in India, but moves away from the usual focus on women and children to a broader view of the entire population. It estimates the economic losses resulting from ignoring under nutrition in the adult working population and questions the current narrow focus of nutrition interventions, suggesting that a family-based approach may provide quicker results and long-term sustainability. It compares the best and worst performing states in the country to glean learnings from both successes and failures and emphasizes the need to hand over the ownership of nutrition outcomes from the state to the community and family for more sustainable results. The book is organized in three sections: Part 1 details the nutrition status of the population, regional variations in nutrition outcomes and government response in terms of interventions. Part 2 reviews issues and concerns like gender discrimination, poor child nutrition status, ineffective implementation of government programmes in the field and the possible impacts of emerging issues like climate change. Part 3 seeks solutions from both international and country experiences.
