Under this title we at once recognize one of the most frequent and formidable surgical diseases of tropical and sub-tropical regions. A comparison of the statistics of our hospital with those of similar institutions in the Antilles and in Africa, where the influence of equatorial conditions upon racial pathology can be studied with greatest advantage, is of interest in showing that civilization and climate have diminished the universally admitted immunity which the negro enjoys from intestinal infections in his own habitat. Knowing, as we all do, that malaria and infective diarrhoea and dysentery are the most potent predisposing causes of suppurative hepatitis, it will be well to glance at the status of the negro race of the present generation with reference to their liability to these diseases. The negro once could boast of his unsusceptibility to malaria and live secure in regions fatal to the white man. This is still true in Africa; but in this country this exemption has been growing less and less complete, and to-day the colored mortality from miasmatic diseases is very much greater than it once was (Corson). From 1898 onward, the expansion of American militarism and empire abroad increasingly relied on black labor, even as policy remained inflected both by scientific racism and by fears of contagion. Black men and women were mobilized for service in the Spanish-Cuban-American War under the War Department's belief that southern blacks carried an immunity against tropical diseases. Later, in World Wars I and II, black troops were stigmatized as members of a contagious "venereal race" and were subjected to experimental medical treatments meant to curtail their sexual desires. This social and cultural history of Civil War medicine and science sheds important light on the question of why and how anti-Black racism survived the destruction of slavery. During the war, white Northerners promoted ideas about Black inferiority under the guise of medical and scientific authority. In particular, the Sanitary Commission and Army medical personnel conducted wartime research aimed at proving Black medical and biological inferiority. They not only subjected Black soldiers and refugees from slavery to substandard health care but also scrutinized them as objects of study. This mistreatment of Black soldiers and civilians extended after life to include dissection, dismemberment, and disposal of the Black war dead in unmarked or mass graves and medical waste pits. Simultaneously, white medical and scientific investigators enhanced their professional standing by establishing their authority on the science of racial difference and hierarchy. Drawing on archives of the U.S. Sanitary Commission, recollections of Civil War soldiers and medical workers, and testimonies from Black Americans, Leslie A. Schwalm exposes the racist ideas and practices that shaped wartime medicine and science.
