Desde el año 1986, la Comisión de la Especialidad de Aparato Digestivo de nuestro país, a instancias de la Asociación Española de Ecografía Digestiva(AEED) estableció la obligatoriedad de la ecografía en el programa de formación del residente de Aparato Digestivo. Aunque no está concretado el tiempo necesario de rotación por la Unidad de Ecografía, parece evidente que al ser una técnica muy explorador dependiente, con una curva de aprendizaje relativamente lenta, este tiempo no debería ser inferior a los seis meses. La importancia de una buena formación ecográfica es fundamental para el gastroenterólogo. La disponibilidad y fiabilidad que implica el que el mismo especialista de aparato digestivo realice la exploración ecográfica de sus enfermos cuyos síntomas conoce en detalle, ofrece una capacidad diagnóstica probablemente superior a la que pueda ofrecer otro profesional que lógicamente no tiene los mismos conocimientos clínicos de nuestra especialidad. La elaboración de este Tratado surgió de esta necesidad de formación y actualización ecográfica, que constituye la esencia y el sentido de existir de la AEED y de la conciencia clara de que no existía una publicación que recogiera los diferentes aspectos ecográficos necesarios para un desarrollo integral de nuestra especialidad. El tratado ha sido coordinado en sus diferentes partes por los miembros de la Junta Directiva de la AEED. El Tratado fue concebido con la suficiente amplitud para que sirva como libro de aprendizaje o libro de texto y con una distribución que permite su uso como libro de consulta en la práctica diaria. Consta de una parte diagnóstica de ecografía en modo B, en la que nos pareció obligado incluir dos capítulos generales de ecografía urológica y ginecológica, cuyo conocimiento permite al gastroenterólogo evitar consultas ecográficas la mayor parte de las veces innecesarias a otros especialistas. Un tercer capítulo sobre ecografía pediátrica, pretende informar al gastroenterólogo sobre aspectos especiales de la edad pediátrica, aspectos que nos plantean dudas en la práctica diaria. La segunda parte referida a ecografía Doppler, incluidas u aplicación en el trasplante hepático, se trata con extensión en seis capítulos. La ecografía intervensionista percutánea, se desarrolla en la tercera parte del tratado de una forma eminentemente práctica y comprensible, La última parte del tratado se ha dedicado a la Ultrasonografía endoscópica, cada vez con más auge y trascendencia en nuestra especialidad. Se han incluido en esta parte aspectos novedosos de diagnóstico con minisondas, y un capítulo sobre elastografía ecoendoscópica, técnica que ha demostrado su importancia en el diagnóstico diferencial incruento entre patología inflamatoria y tumoral pancreática, y que sin duda tendrá un importante desarrollo en el futuro.
