The health systems inherited by transition countries of Europe and Central Asia (ECA) are changing in response to fundamental and unprecedented challenges. Although the desired shape of future health systems in many ECA countries is discernible, the process for getting there must be invented along the way. 'A Health Sector Strategy for the Europe and Central Asia Region' reviews substantive issues facing health policymakers in ECA. It summarizes the World Bank's experience so far in this arena and the lessons it suggests. Furthermore, it outlines both an external strategy by which the Bank's ECA health staff could assist countries to restructure their health systems and an internal strategy by which the staff could organize their own activities to achieve this result. The publication offers those outside the Bank a basis for professional dialogue to foster constructive change in the Bank's approaches.
Fifty years ago, health outcomes in the countries of Eastern Europe and Central Asia were not far behind those in Western Europe and well ahead of most other regions of the world. But progress since then has been slow. While life expectancy in the ECA region today is close to the global average, the gap with its western neighbors has doubled, and other middle-income regions have all surpassed ECA. Some countries in the region are doing better, but full convergence with the world’s most advanced health systems is still a long way off. At the same time, survey evidence suggests that the health sector is the top priority for additional investment among populations across the region. The experience of high-income countries also suggests that popular demand for strong and accessible health systems will only grow over time. Yet these aspirations must be reconciled with current fiscal realities. In brief, health sector issues are a challenge here to stay for policy-makers across the ECA region. This report draws on new evidence to explore the development challenge facing health sectors in ECA, and highlights three key agendas to help policy-makers seeking to achieve more rapid convergence with the world’s best performing health systems. The first is the health agenda, where the task is to strengthen public health and primary care interventions to help launch the “cardiovascular revolution” that has taken place in the West in recent decades. The second is the financing agenda, in which growing demand for medical care must be satisfied without imposing undue burden on households or government budgets. The third agenda relates to broader institutional arrangements. Here there are some key reform ingredients common to most advanced health systems that are still missing in many ECA countries. A common theme in each of these three agendas is the emphasis on improving outcomes, or “Getting Better”.

With the larger role that the agricultural sector plays in the Eastern Europe and Central Asian (ECA) region, emphasis must be placed on the sector's sustained growth and prosperity. As the analysis indicates, the region's rural economy is still struggling to adjust to new economic realities, and this will require further refinement and adjustment of the Bank's approach as well. The Europe and Central Asia (ECA) region encompasses great economic, social and environmental diversity, and its countries are facing various food security and nutrition challenges. While they have made significant progress in reducing the prevalence of undernourishment over the past two decades, new evidence shows a stagnation of this trend, particularly in Central Asia. Malnutrition in one or more of its three main forms – undernutrition, micronutrient deficiencies, and overweight and obesity – is present to varying degrees in all countries of the region. Often, all three forms coexist, creating what is called the “triple burden of malnutrition.” Overweight among children and obesity among adults continue to rise – with now almost one-fourth of the region’s adults obese – and constitute a significant concern for future health and well-being and related costs. While poverty levels in most ECA countries have been declining in recent years, poverty coupled with inequality has led to increased vulnerability of disadvantaged groups and populations in rural and remote areas of low- and lower-middle-income countries. New analysis shows that adult women have a higher prevalence of severe food insecurity than men in some areas, pointing to gender inequalities that are reflected in access to food. Addressing gender and other inequalities is key to achieving the Sustainable Development Goals and heeding the call to “leave no one behind.” The publication’s focus this year is on migration, gender and youth and the linkages with rural development and food security in Europe and Central Asia. Migration is linked in multiple ways to gender, youth, and agricultural and rural development – both as a driver and possible source of development opportunities, with labour migration and remittances playing significant roles in the region. Changing migration processes need to be fully understood to better address the challenges of migration and harness the potential benefits for sustainable development and revitalized rural areas. Governments, public and private institutions, communities and other concerned parties must strengthen collaboration and scale up efforts towards achieving the goals of a thriving, healthy and food-secure region.

This study evaluates the effectiveness of the World Bank Group's support for health, nutrition, and population (HNP) in developing countries from 1997 to 2008 - totaling more than $17 billion - and distills lessons for greater impact in the future. It finds that the Bank Group now funds a smaller share of global support for HNP than a decade ago, but its support remains substantial and adds considerable value. About two-thirds of the Bank's HNP support has had satisfactory outcomes, often in difficult environments. But in a number of country settings, particularly in Africa, it has not performed well, in part due to high complexity and weak capacity. Only half of HNP support had a pro-poor focus, while support to reduce high fertility and promote family planning has dwindled. The evaluation highlights the contribution of investments in water supply, sanitation, and hygiene to improving the health of the poor and the lessons from support for sector-wide approaches, communicable disease control, and health reform. Moving forward, the World Bank needs to improve the performance of its HNP support and the Bank and IFC need to take actions to ensure their support reaches the poor and contributes greater social benefits, respectively.
