Between 1942 and 1975, the U.S. Army conducted tests with human subjects to study the effects of a variety of agents, including chemical warfare and biological agents. The potential long-term health effects on the test subjects from their exposures have been evaluated periodically, most recently in a report titled Assessment of Potential Long-Term Health Effects on Army Human Test Subjects of Relevant Biological and Chemical Agents, Drugs, Medications and Substances: Literature Review and Analysis (the Report), which was prepared by a contractor to assist the Army with making determinations about providing medical care to former test subjects. In response to a request by the Army, the National Academies of Sciences, Engineering, and Medicine formed a committee that was tasked with examining whether the Report appropriately identified potential long-term health effects from exposure to the test agents and whether an adequate weight-of-evidence approach was used to characterize the strength of the associations between the agents and their potential health effects. The committee was made aware at its first meeting on November 30, 2017, that the Army had already begun to receive applications for medical care and that some determinations may need to be made before the committee's evaluation of the Report was completed. Because of this urgency, the Army developed a process by which applications for medical care will be reviewed, and as a result, the committee was given the additional task of reviewing the Army's Memorandum that describes the approach that will be used by the Army to evaluate agent- and outcome-specific associations. This interim report was prepared to facilitate the Army's deliberations. A review of the Report is presented first, followed by a review of the Memorandum.

From the editor: "71F, or "71 Foxtrot," is the AOC (area of concentration) code assigned by the U.S. Army to the specialty of Research Psychology. Qualifying as an Army research psychologist requires, first of all, a Ph.D. from a research (not clinical) intensive graduate psychology program. Due to their advanced education, research psychologists receive a direct commission as Army officers in the Medical Service Corps at the rank of captain. In terms of numbers, the 71F AOC is a small one, with only 25 to 30 officers serving in any given year. However, the 71F impact is much bigger than this small cadre suggests. Army research psychologists apply their extensive training and expertise in the science of psychology and social behavior toward understanding, preserving, and enhancing the health, well being, morale, and performance of Soldiers and military families. As is clear throughout the pages of this book, they do this in many ways and in many areas, but always with a scientific approach. This is the 71F advantage: applying the science of psychology to understand the human dimension, and developing programs, policies, and products to benefit the person in military operations. This book grew out of the April 2008 biennial conference of U.S. Army Research Psychologists, held in Bethesda, Maryland. This meeting was to be my last as Consultant to the Surgeon General for Research Psychology, and I thought it would be a good idea to publish proceedings, which had not been done before. As Consultant, I'd often wished for such a document to help explain to people what it is that Army Research Psychologists "do for a living." In addition to our core group of 71Fs, at the Bethesda 2008 meeting we had several brand-new members, and a number of distinguished retirees, the "grey-beards" of the 71F clan. Together with longtime 71F colleagues Ross Pastel and Mark Vaitkus, I also saw an unusual opportunity to capture some of the history of the Army Research Psychology specialty while providing a representative sample of current 71F research and activities. It seemed to us especially important to do this at a time when the operational demands on the Army and the total force were reaching unprecedented levels, with no sign of easing, and with the Army in turn relying more heavily on research psychology to inform its programs for protecting the health, well being, and performance of Soldiers and their families."

In the spring of 2010, CSIS launched a year-long, independent examination of the U.S. Army and Navy overseas medical research laboratories. The impetus was an awareness that despite the laboratories' impressive scientific accomplishments and contributions to U.S. national interests and global health, they are not well understood outside of research circles and consequently find themselves undervalued in today's environment of fiscal austerity. They stand at the intersection of health and security, a topic of increased importance to U.S. approaches to global health. The CSIS project aimed to assess the laboratories' contributions and achievements; examine the factors that constrain their performance; and propose reforms that will put them on the best course to continued success. It included considerable background research, three formal meetings of experts, travel to five overseas laboratories, and interviews with dozens of laboratory researchers and collaborators. This report lays out the project's research, conclusions, and recommendations. The laboratories contribute fundamentally to U.S. military readiness, their core function, through medical research tied directly to the protection of deployed personnel. In addition, their development of products such as vaccines, therapies, medical devices and new prophylactic drugs brings shared health benefits to the world at large, strengthens the scientific community in the host country, and attrafts other U.S. government agencies as well as multiple university, business and foundation partners, eager to advance field trials and other scientific research in an endemic disease environment. At the same time, the laboratories contribute substantially to related scientific activities, most notably global detection of emerging and re-emerging disease threats, while helping to build military-to-military collaboration and local medical and scientific capacity. They are also active and valued participants in U.S. Embassy country teams, the in-country face of U.S. diplomacy.

The charge of the Army Research Laboratory Technical Assessment Board (ARLTAB) is to provide biannual assessments of the scientific and technical quality of the research, development, and analysis programs at the Army Research Laboratory (ARL). The advice provided in this report focuses on technical rather than programmatic considerations. The Board is assisted by six National Research Council (NRC) panels, each of which focuses on the portion of the ARL program conducted by one of ARL's six directorates. When requested to do so by ARL, the Board also examines work that cuts across the directorates. The Board has been performing assessments of ARL since 1996. The current report summarizes its finding for the 2009-2010 period, during which 96 volunteer experts in fields of science and engineering participated in the following activities: visiting ARL annually, receiving formal presentations of technical work, examining facilities, engaging in technical discussions with ARL staff, and reviewing ARL technical materials. The Board continues to be impressed by the overall quality of ARL's technical staff and their work and applauds ARL for its clear, passionate concern for the end user of its technology-the soldier in the field-and for ARL's demonstrated mindfulness of the importance of transitioning technology to support immediate and longer-term Army needs. ARL staff also continue to expand their involvement with the wider scientific and engineering community. In general, ARL is working very well within an appropriate research and development (R&D) niche and has been demonstrating significant accomplishments.

To guide mission planning, military decision makers need information on the health risks of potential exposures to individual soldiers and their potential impact on mission operations. To help with the assessment of chemical hazards, the U.S. Army Center for Health Promotion and Preventive Medicine developed three technical guides for characterizing chemicals in terms of their risks to the mission and to the health of the force. The report reviews these guides for their scientific validity and conformance with current risk-assessment practices. The report finds that the military exposure guidelines are appropriate (with some modification) for providing force health protection, but that for assessing mission risk, a new set of exposure guidelines is needed that predict concentrations at which health effects would degrade the performance of enough soldiers to hinder mission accomplishment.
