Over the past two decades, Biosafety in Microbiological and Biomedical Laboratories (BMBL) has become the code of practice for biosafety—the discipline addressing the safe handling and containment of infectious microorganisms and hazardous biological materials. The principles of biosafety introduced in 1984 in the first edition of BMBL and carried through in this fifth edition remain steadfast. These principles are containment and risk assessment. The fundamentals of containment include the microbiological practices, safety equipment, and facility safeguards that protect laboratory workers, the environment, and the public from exposure to infectious microorganisms that are handled and stored in the laboratory. Risk assessment is the process that enables the appropriate selection of microbiological practices, safety equipment, and facility safeguards that can prevent laboratory-associated infections (LAI). The purpose of periodic updates of BMBL is to refine guidance based on new knowledge and experiences and to address contemporary issues that present new risks that confront laboratory workers and the public health. In this way the code of practice will continue to serve the microbiological and biomedical community as a relevant and valuable authoritative reference. Since its publication by the National Institutes of Health (NIH) and the Centers for Disease Control and Prevention (CDC) in 1984, Biosafety in Microbiological and Biomedical Laboratories (BMBL) has become the cornerstone of the practice of biosafety in the United States and in many countries around the world. The BMBL has been revised periodically over the past three decades to refine the guidance it provides based on new knowledge and experiencesâ€"allowing it to remain a relevant, valuable, and authoritative reference for the microbiological and biomedical community. Seven years after the release of the BMBL 5th Edition, NIH and CDC are considering a revision based on the comments of a broader set of stakeholders. At the request of NIH, the National Academies of Sciences, Engineering and Medicine conducted a virtual town hall meeting from 4 April to 20 May 2016 to allow BMBL users to share their thoughts on the BMBL in general and its individual sections and appendices. Specifically, users were asked to indicate what information they think should be added, revised, or deleted. Major themes from the virtual town hall meeting were further discussed in a workshop held on 12 May 2016 in Washington, DC. This document encapsulates the discussion of the major comments on the BMBL that were posted on the virtual town hall prior to 12 May 2016 and the various BMBL comments and issues related to biosafety that were raised during the workshop by participants who attended the meeting in Washington DC and those who listened to the live webcast.

During July 10-13, 2011, 68 participants from 32 countries gathered in Istanbul, Turkey for a workshop organized by the United States National Research Council on Anticipating Biosecurity Challenges of the Global Expansion of High-containment Biological Laboratories. The United States Department of State's Biosecurity Engagement Program sponsored the workshop, which was held in partnership with the Turkish Academy of Sciences. The international workshop examined biosafety and biosecurity issues related to the design, construction, maintenance, and operation of high-containment biological laboratories- equivalent to United States Centers for Disease Control and Prevention biological safety level 3 or 4 labs. Although these laboratories are needed to characterize highly dangerous human and animal pathogens, assist in disease surveillance, and produce vaccines, they are complex systems with inherent risks. Biosecurity Challenges of the Global Expansion of High-Containment Biological Laboratories summarizes the workshop discussion, which included the following topics: Technological options to meet diagnostic, research, and other goals; Laboratory construction and commissioning; Operational maintenance to provide sustainable capabilities, safety, and security; and Measures for encouraging a culture of responsible conduct. Workshop attendees described the history and current challenges they face in their individual laboratories. Speakers recounted steps they were taking to improve safety and security, from running training programs to implementing a variety of personnel reliability measures. Many also spoke about physical security, access controls, and monitoring pathogen inventories. Workshop participants also identified tensions in the field and suggested possible areas for action.
