Improvements and efficiencies have been made in key areas of cancer care since the Cancer Reform Strategy was published in 2007. The NAO estimates that cancer cost the NHS approximately £6.3 billion in 2008-09, but it is not clear if the implementation of the Strategy is achieving value for money. Reported spending on cancer care varies between PCTs - in 2008-09 varying from £55 to £154 per head - and there is unexplained variation from year to year. Significant reductions have been made in the number of days cancer patients spend in hospital - largely as a result of increasingly treating patients as day cases. The Strategy aimed to minimise emergency admissions for cancer patients, but these are still increasing, with wide variations between PCTs and poor understanding of the reasons for those variations. There are opportunities to achieve better outcomes and free up resources. Reducing the average length of stay in hospital to the level of the best performing PCTs, efficiencies worth some £113 million a year could be achieved. If the number of inpatient admissions per new cancer diagnosis was reduced to the level of the best performing PCT, bed days equivalent to around £106 million each year could be saved. Radiotherapy machines could be used more productively to help the NHS meet increasing demand. High quality information is essential to be able to commission services successfully and to monitor performance. Some information on cancer has improved, but significant gaps still remain.

This report examines the Department of Health's and the NHS's performance in delivering cancer services; improving information on activity, cost and outcomes of cancer services; and how the Department intends to deliver cost-effective cancer services in the new NHS. The NHS spent £6.3 billion on cancer services in 2008-09. Tackling Cancer has been a priority for the Department since its ten year NHS Cancer Plan was published in 2000. In 2007 the Department published its five year Cancer Reform Strategy (the Strategy) to deliver improved patient outcomes. The NHS has made significant progress in delivering important aspects of cancer services, with falling mortality rates and consistent achievement of the cancer waiting times targets. However, early diagnosis does not happen often enough. And the gap in survival rates between England and the best European countries has not been closed. There remain wide, unexplained variations in the performance of cancer services and in the types of treatment available across the country; and significant gaps in information about important aspects of cancer services, in particular information on chemotherapy, on follow-up treatment, and on the stage that a patient's cancer has reached at the time of diagnosis. The Department cannot yet measure the impact of the Strategy on key outcomes, such as survival rates, and does not know if cancer services are being commissioned cost-effectively, due to poor data on costs and because outcomes data are not sufficiently timely. The Department must ensure the collection of high quality, comprehensive and timely data.

In the United States, approximately 14 million people have had cancer and more than 1.6 million new cases are diagnosed each year. However, more than a decade after the Institute of Medicine (IOM) first studied the quality of cancer care, the barriers to achieving excellent care for all cancer patients remain daunting. Care often is not patient-centered, many patients do not receive palliative care to manage their symptoms and side effects from treatment, and decisions about care often are not based on the latest scientific evidence. The cost of cancer care also is rising faster than many sectors of medicine--having increased to $125 billion in 2010 from $72 billion in 2004--and is projected to reach $173 billion by 2020. Rising costs are making cancer care less affordable for patients and their families and are creating disparities in patients' access to high-quality cancer care. There also are growing shortages of health professionals skilled in providing cancer care, and the number of adults age 65 and older--the group most susceptible to cancer--is expected to double by 2030, contributing to a 45 percent increase in the number of people developing cancer. The current care delivery system is poorly prepared to address the care needs of this population, which are complex due to altered physiology, functional and cognitive impairment, multiple coexisting diseases, increased side effects from treatment, and greater need for social support. Delivering High-Quality Cancer Care: Charting a New Course for a System in Crisis presents a conceptual framework for improving the quality of cancer care. This study proposes improvements to six interconnected components of care: (1) engaged patients; (2) an adequately staffed, trained, and coordinated workforce; (3) evidence-based care; (4) learning health care information technology (IT); (5) translation of evidence into clinical practice, quality measurement and performance improvement; and (6) accessible and affordable care. This report recommends changes across the board in these areas to improve the quality of care. Delivering High-Quality Cancer Care: Charting a New Course for a System in Crisis provides information for cancer care teams, patients and their families, researchers, quality metrics developers, and payers, as well as HHS, other federal agencies, and industry to reevaluate their current roles and responsibilities in cancer care and work together to develop a higher quality care delivery system. By working toward this shared goal, the cancer care community can improve the quality of life and outcomes for people facing a cancer diagnosis.

The cost effective delivery of armoured vehicle capability by Great Britain: Parliament: House of Commons: Committee of Public Accounts Pdf

Armoured vehicles such as tanks, reconnaissance and personnel-carrying vehicles are essential for a wide range of military tasks. Since the 1998 Strategic Defence Review, the Ministry of Defence has attempted to acquire the vehicles it needs through a number of procurement projects. However, none of the principal armoured vehicles it requires have yet been delivered, despite the MoD spending £1.1 billion since 1998, including £321 million wasted on cancelled or suspended projects. As a result there will be gaps in capability until at least 2025, making it more difficult to undertake essential tasks such as battlefield reconnaissance. Partly as a result of this £1.1 billion failure to yet deliver any armoured vehicles, and to meet the specific military demands of operating in Iraq and Afghanistan, the MoD was provided with a further £2.8 billion from the Treasury Reserve to buy Urgent Operational Requirements (UOR) vehicles. Over the past six years, the Department has removed £10.8 billion from armoured vehicle budgets up to 2021. This has left £5.5 billion available for the next ten years, which is insufficient to deliver all of the armoured vehicle programmes which are planned. The MoD needs to be clearer about its priorities, and stop raiding the armoured vehicles chest every time it needs to make savings across the defence budget. It will also need to set more realistic requirements in future if it is to deliver projects on time and to budget. The Committee expressed concern that the Department was unable to identify anyone who has been held to account for the clear delivery failures. Further, the MoD has yet to balance its defence budget fully and devise a plan to close capability gaps, despite having conducted the SDSR and two subsequent planning exercises. It needs to determine its armoured vehicle equipment priorities and deliver these as rapidly and cost-effectively as possible, including making an assessment of which of its existing vehicles should be retained after combat operations in Afghanistan cease.

The Efficiency and Reform Group (the Group) was established within the Cabinet Office in May 2010 to lead efforts to cut government spending by £6 billion in 2010-11. Its long term aim is to improve value for money across government by strengthening the central coordination of measures to improve efficiency. The imperative to make savings in the short term has involved the Group imposing new controls on departments, such as moratoria on certain expenditure. Sustained efficiency improvements, though, will need a much deeper change to both the culture and institutional structure of government. The Group also needs to clear up confusion over who is accountable for what in terms of improving value for money, especially in defining its responsibilities and those of the Treasury and individual departments. The Group's actions have resulted in efficiency savings of £3.75 billion across departments in 2010-11. It should continue to describe any future spending reductions accurately and explain any impact on services. The scale of the challenge to deliver efficiencies is huge: the Government intends that half of the £81 billion reduction in spending planned over the next three years should come from efficiencies rather than through cuts to services or delays to important projects. Many of the efficiencies must be achieved in areas where the Group currently has a limited influence, or by local bodies, where it has none. The Group should set out how it will operate to ensure that its approach can be replicated across the wider public sector.

When the 2010 Strategic Defence and Security Review (SDSR) had started, the Department had contracts for two carriers with an estimated cost of £5.24 billion and delivery dates of 2016 and 2018. Decisions taken in the Review mean the UK will have no carrier aircraft capability from 2011-2020. While two carriers are still being built, only one will be converted to launch the planes that have now been selected, and the other will be mothballed. The UK will only have one operational carrier with a significantly reduced availability at sea when Carrier Strike capability is reintroduced in 2020. That carrier is being built according to the old design and will have to be modified to make it compatible with the requirements of the new aircraft: the cost of these modifications will not be known until 2012. The SDSR decision is forecast to save £3.4 billion, but only £600 million of this is cash savings while the remainder is simply deferring expenditure beyond the Department's 10 year planning horizon. The decision will lead to nine years without Carrier Strike and full capability will not be achieved until 2030. And more work will be needed to get the best and most flexible operational use from the carrier. The Committee is disappointed that the systemic issues that have appeared in its other recent defence reports continue to arise. The Committee has built on what has been said in past reports and focussed on two key areas: strategic decision-making and delivery of capabilities

The National Programme for IT in the NHS is an ambitious £11.4 billion programme of investment. This report is concerned with a central part of the Programme, where the aim was to create a fully integrated electronic care records system, which is expected to cost around £7 billion in total The Department has failed to demonstrate the benefits achieved for the £2.7 billion spent to date on care records systems and has accepted it is unable to deliver its original vision. It is now relying on individual NHS trusts to develop systems compatible with those in the Programme. Furthermore the Department could not explain how potential inconsistencies would be dealt with or what it will cost local NHS organisations to connect up. The Department has not got the best out of its suppliers, despite having paid them some £1.8 billion so far. One supplier, CSC, has yet to deliver the bulk of the systems it is contracted to supply and has instead implemented a large number of interim systems as a stopgap. The Department has been in negotiations with CSC for over a year, but conceded that it may be more expensive to terminate the contract than to complete it. The Department has also revised its contract with BT reducing the number of systems and increasing the price for each system delivered. This has resulted in BT being paid £9 million to implement systems at each NHS site, even though the same systems have been purchased for under £2 million by NHS organisations outside the Programme. The Committee is further concerned about the problems in getting timely and reliable information from the Department. Information provided has frequently been late, has contained inconsistencies and has contradicted other evidence.
