Fluid Resuscitation by Institute of Medicine,Committee on Fluid Resuscitation for Combat Casualties Pdf

Historically, 20% of all injured combatants die on the battlefield before they can be evacuated to a field hospital. Blood lossâ€"hemorrhageâ€"is the single major cause of death among those killed in action whose lives might otherwise be saved. Fluid resuscitation and the treatment of hypovolemia (the abnormally decreased volume of circulating fluid in the body) offer the greatest opportunity for reducing mortality and morbidity associated with battlefield casualties. In Fluid Resuscitation, a committee of experts assess current resuscitation fluids and protocols for the treatment of combat casualties and make recommendations for future research. Chapters focus on the pathophysiology of acute hemorrhagic shock, experience with and complications of fluid resuscitation, novel approaches to the treatment of shock, protocols of care at the site of injury, and future directions for research. The committee explicitly describes the similarities and differences between acute medical care during combat and civilian emergency trauma care. Fluid Resuscitation should help energize and focus research in both civilian and military emergency care and help save the lives of citizens and soldiers alike.

Perioperative Fluid Management by Ehab Farag,Andrea Kurz,Christopher Troianos Pdf

This revised and expanded second edition presents the most recent evidence-based facts on perioperative fluid management and discusses fluid management from basic sciences to clinical applications and the patients’ outcomes. Recent advances in understanding the Revised Starling principle with new concepts in tissue perfusion and the most recent techniques of perioperative goal directed fluid management are described. The endothelial glycocalyx functions and the influence of fluid management on its integrity are covered in detail; moreover, the techniques for its protection are also discussed. The dilemma of perioperative use of hydroxyethyl starch solutions and the resurgence of interest in using human albumin as an alternative colloid is explored. The problems of using unbuffered solutions during the perioperative period and comparison between restrictive versus liberal fluid management are discussed in full. Lastly, case scenarios for every possible clinical situation describe the most up-to-date fluid management for the corresponding clinical problem. Perioperative Fluid Management, Second Edition is of interest to anesthesiologists and also intensivists.

Body Fluid Management by Felice Agrò Pdf

The administration of intravenous fluids is one of the most common and important therapeutic practices in the treatment of surgical, medical and critically ill patients. The international literature accordingly contains a vast number of works on fluid management, yet there is still confusion as to the best options in the various situations encountered in clinical practice. The purpose of this volume is to help the decision-making process by comparing different solution properties describing their indications, mechanisms of action and side-effects according to physiologic body water distribution, electrolytic and acid-base balance, and to clarify which products available on the market represent the best choice in different circumstances. The book opens by discussing in detail the concepts central to a sound understanding of abnormalities in fluid and electrolyte homeostasis and the effect of intravenous fluid administration. In the second part of the monograph, these concepts are used to explain the advantages and disadvantages of solutions available on the market in different clinical settings. Body Fluid Management: From Physiology to Therapy will serve as an invaluable decision-making guide, including for those who are not experts in the subject.

Handbook of ICU Therapy by Ian McConachie Pdf

Handbook of ICU Therapy provides rapid access to important information on the treatment of the critically ill patient. It comprises a series of 'cutting edge' reviews of the most advanced treatment concepts available in the modern ICU. Whilst assuming a basic knowledge of underlying conditions, it nonetheless outlines key physiological principles where necessary, and critically reviews current literature and best practice. The heart of the book is aimed at providing key practical information on treatment techniques to the busy clinician in an easily accessible style. In addition to conventional drug therapy, ventilator, fluid and physical therapies are also discussed in detail. All of the authors are directly involved in ICU research and practice and are familiar with all of the latest developments in this fast-moving field of medicine.

Fluid Physiology by Thomas Woodcock Pdf

This book is essential reading for medical or veterinary practitioners who need to understand the new fluid physiology and to apply it to the safe care of patients. The Starling principle is one of medicine’s most important concepts and originates from Ernest Starling’s laboratory research 120 years ago. However, inappropriate fluid therapy is now recognised as harming and even killing thousands of patients every year. In 2004, a landmark study was published which confirmed the hypotheses put forward by Sheldon Weinbaum and Charles Michel, among other physiologists, that, in most tissues and in most situations, capillaries filter fluid to the interstitium, but do not reabsorb it. This book draws together for the first time the evolving science of the steady-state Starling principle and the clinical evidence that reveals its applicability to safer patient care. It is a thorough re-appraisal of the basics of fluid therapy. The mantra of colloid boluses for plasma volume resuscitation and colloid-free isotonic salt solution for extracellular fluid volume does not explain observations from blinded clinical trials, and the expectation of benefit for resuscitation with colloids, particularly in respect of oedema, has not materialised. Now that there is consensus that colloid volume therapy should not be used in critically-ill patients, there is a pressing need for a new paradigm for fluid therapy. This book proposes an improved paradigm that takes into consideration the Starling principle, which has been neglected by clinicians and revised by physiologists in recent years. It retires the view of colloids as preferred plasma substitutes, and focuses instead on the central volume of distribution of an infused fluid, its rate of distribution to a peripheral volume, and its rate of excretion. In short, it emphasises volume kinetics.

Damage Control Resuscitation by Philip C. Spinella Pdf

This book provides a comprehensive overview of damage control resuscitation (DCR), an evidence-based approach to the resuscitation of patients with severe life-threatening hemorrhage (LTH). It focuses on both civilian and military applications as DCR is utilized in civilian trauma situations as well as combat casualty care settings. The book covers the history of fluid resuscitation for bleeding, epidemiology of severe traumatic injuries, prediction of life-threatening hemorrhage, pathophysiology and diagnosis of blood failure, and permissive hypotension. Chapters provide in-depth detail on hemostatic resuscitation principles, dried plasma, dried platelet surrogates, and recent developments in frozen red blood cells and oxygen carriers. The book also discusses how DCR principles can be used in a variety of situations such as when there are large numbers of patients with hemorrhagic lesions, non-trauma scenarios, and on distinct populations such as children. Finally, it concludes with a discussion of training and education methods for the implementation of DCR and remote DCR principles as well as learning healthcare system principles to facilitate the implementation of DCR and ultimately improve outcomes for patients with life-threatening hemorrhage. Damage Control Resuscitation: Identification and Treatment of Life-Threatening Hemorrhage is an essential resource for physicians and related professionals, residents, nurses and medical students in emergency medicine, anesthesia, surgery, and critical care, as well as civilian and military EMS providers.
