Human and Organisational Factors by Benoît Journé,Hervé Laroche,Corinne Bieder,Claude Gilbert Pdf

This open access book addresses several questions regarding the implementation of human and organisational factors (HOF) so that recent improvements in industrial safety can be built upon. It addresses sources of frustration in senior management with high expectations of operational recommendations and disquiet on the part of HOF specialists struggling to have an impact on high-level decision making. The brief explores these issues with an emphasis on examples and lessons learned based on the experience of its authors, who come from different academic disciplines and various industrial sectors such as oil and gas, energy and transportation. It then offers some ways forward for a better consideration of HOF in hazardous companies with a view of promoting safety and facing challenges in a rapidly changing world.

Applied Operational Excellence for the Oil, Gas, and Process Industries by Dennis P. Nolan,Eric T Anderson Pdf

Applied Operational Excellence for the Oil, Gas, and Process Industries offers a straightforward practical guide for oil and gas companies to understand the comparisons and contrasts between various types of safety management processes, including the standardized structure and ongoing extended benefits that operational excellence can bring to an oil and gas company. The goal of achieving operational excellence is to reduce costs, improve productivity, and enhance efficiency—in other words, operational excellence contributes to the bottom line. Following along with pre-built success in the process industries, many companies in the oil and gas industry appear to use a subset form of operational excellence, yet many are unsure or unaware of all the safety system components that will truly benefit the company holistically, and current literature is only applicable to the process and manufacturing industries. Packed with clear objectives and tools, structure guidelines specific to oil and gas, and guidance for how to imbed your existing safety program under the operational excellence umbrella known as "One-Step Merger," this book will help you establish an overall safety culture vision and challenge your organization to achieve higher levels of safety management and overall company value. Explores how to solidify a foundational operational excellence program applicable for your oil and gas company Clarifies the differences and benefits among various programs under operational excellence (OE), such as SHE (safety, health, and environment), PSM (process safety management), and SMS (safety management system) Explains how to audit and consistently assess how oil and gas OE systems are planned, implemented, and managed, with explanations on cost and time impacts as well as administrative protocols Includes a glossary, acronym appendix, and additional references for further reading

The Buncefield Investigation by Great Britain. Department for Work and Pensions Pdf

The explosions at Buncefield Oil Storage Depot on 11 December 2005 resulted in injuries to more than 40 people and significant damage to the surrounding area and the environment. This publication is a response by the DWP and the Competent Authority (the Health and Safety Executive, Environment Agency, Scottish Environment Protection Agency) to the first two reports from the Buncefield Major Incident Investigation Board (MIIB) on (1) the design and operation of fuel storage sites and (2) emergency preparedness for, response to and recovery from major incidents, which may be viewed at http://www.buncefieldinvestigation.gov.uk/index.htm. Overall, good progress has been made against the Board's recommendations both at fuel storage sites as well as other Control of Major Accident Hazards (COMAH) sites. Some of the recommendations set a higher standard than is generally in place in the sector, and their introduction needs to be planned and phased in with care, not least to avoid compromising the resilience of the UK's fuel supply. On the second report, a programme of action has been devised and positive work at local level has enhanced the resilience and effectiveness of emergency response. This response outlines progress against recommendations in each report.

Health and Safety in a Changing World by Robert Dingwall,Shelley Frost Pdf

When health and safety regulatory frameworks took their present form in the 1970s, they were seen as a triumph of welfare state intervention. Since then, as heavy industry has declined and office and retail employment have expanded, new ways of working have radically altered the context of health and safety policy. Many people have come to see health and safety interventions as an obstacle to innovation. This book aims to address the changing context of health and safety policy, exploring concerns arising within the profession and the appropriate responses. Its manifesto for reform promises to frame the debate within the professional and policy community for a generation. The result of a major research programme funded by the Institution of Occupational Safety and Health (IOSH), Health and Safety in a Changing World shows how health and safety policy has developed over time, how it is applied in practice and how best to make it fit-for-purpose in the 21st century. The book will be essential reading for professionals, practitioners and academic readers with an interest in the rapidly-evolving field of health and safety.

Developing an Effective Safety Culture by James Roughton,James Mercurio Pdf

Developing an Effective Safety Culture implements a simple philosophy, namely that working safely is a cultural issue. An effective safety culture will eventually lead to the desired goal of zero incidents in the work place, and this book will provide an understanding of what is needed to reach this goal. The authors present reference material for all phases of building a safety management system and ultimately developing a safety program that fits the culture. This volume offers the most comprehensive approach to developing an effective safety culture. Information is easily accessible as the authors move first through, understanding the cost of incidents, then to perspectives and descriptions of management systems, principal management leadership traits, establishing and evaluating goals and objectives, providing visible leadership, and assigning required responsibilities. In addition, you are given the means to systematically identifying hazards and develop your own hazard inventory and control system. Further information on OSHA requirements for training, behavior-based safety processes, and the development of a job hazard analysis for each task is available as well. Valuable case studies, from the authors' own experience in the industry, are used throughout to demonstrate the concepts presented. * Provides the tools to rebuild or enhance a desired safety culture * Allows you to identify a program that will fit your specific application * Examines different philosophies in relation to safety culture development
