Today’s era of intense globalization has unleashed dynamic movements of people, pathogens, and pests that overwhelm the static territorial jurisdictions on which the governance provided by sovereign states and their formal intergovernmental institutions is based. This world of movement calls for new ideas and institutions to govern people’s health, above all in Africa, where the movements and health challenges are the most acute. This book insightfully explores these challenges in ways that put the perspectives of Africans themselves at centre stage. It begins with the long central and still compelling African health challenge of combating the pandemic of HIV/AIDS. It then examines the global governance responses by the major multilateral organizations of the World Bank and the World Trade Organization and the newer informal flexible democratically oriented ones of the Group of Eight. It also addresses the compounding health challenge created by climate change to assess both its intensifying impact on Africa and how all international institutions have largely failed to link climate and health in their governance response. It concludes with several recommendations about the innovative ideas and institutions that offer a way to closing the great global governance gaps and thus improving Africans’ health and that of citizens beyond.

In the United States, some populations suffer from far greater disparities in health than others. Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. The most recent Ebola epidemic that began in late 2013 alerted the entire world to the gaps in infectious disease emergency preparedness and response. The regional outbreak that progressed to a significant public health emergency of international concern (PHEIC) in a matter of months killed 11,310 and infected more than 28,616. While this outbreak bears some unique distinctions to past outbreaks, many characteristics remain the same and contributed to tragic loss of human life and unnecessary expenditure of capital: insufficient knowledge of the disease, its reservoirs, and its transmission; delayed prevention efforts and treatment; poor control of the disease in hospital settings; and inadequate community and international responses. Recognizing the opportunity to learn from the countless lessons of this epidemic, the National Academies of Sciences, Engineering, and Medicine convened a workshop in March 2015 to discuss the challenges to successful outbreak responses at the scientific, clinical, and global health levels. Workshop participants explored the epidemic from multiple perspectives, identified important questions about Ebola that remained unanswered, and sought to apply this understanding to the broad challenges posed by Ebola and other emerging pathogens, to prevent the international community from being taken by surprise once again in the face of these threats. This publication summarizes the presentations and discussions from the workshop.

In a country as diverse as South Africa, sickness and health often mean different things to different people – so much so that the different health definitions and health belief models in the country seem to have a profound influence on the health-seeking behaviour of the people who are part of our vibrant, multicultural society. This book is concerned with the integration of indigenous health knowledge (IHK) into the current Western--orientated Primary Health Care (PHC) model. The first section of the book highlights the challenges facing the training of health professionals using a curriculum that is not drawing its knowledge base from the indigenous context and the people of that context. Such professionals will later recognise that they are walking without limbs in matters pertaining to health. The area that was chosen for conducting the research was KwaBomvana in Xhora (Elliotdale), Eastern Cape province, South Africa. The people who reside there are called AmaBomvana. The area where the Bomvana peoples reside is served by Madwaleni Hospital and eight surrounding clinics. Qualitative ethnographic, feminist methods of data collection supported the research done for Section 1 of the book. Section 2 comprises the translation and implementation of PhD study outcomes and had contributions from various researchers. In the critical research findings of the PhD study, older Xhosa women identify the inclusion of social determinants of health as vital to the health problems they managed within their homes. For them, each disease is linked to a social determinant of health, and the management of health problems includes the management of social determinants of health. For them, it is about the health of the home and not just about the management of disease. They believe that healthy homes make healthy villages, and that the prevention of the development of disease is related to the strengthening of the home. Health and illness should be seen within both physical and spiritual contexts; without health, there can be no progress in the home. When defining health, the older Xhosa women add three critical components to the WHO health definition, namely, food security, healthy children and families, and peace and security in their villages. Prof. Mji further proposes that these three elements should be included in the next revision of the WHO health definition because they are not only important for the Bomvana people where the research was conducted, but also for the rest of humanity. In light of the promise of National Health Insurance and the revitalisation of PHC, this book proposes that these two major national health policies should take cognisance of the IHK utilised by the older Xhosa women. In addtion to what this research implies, these policies should also take note of all IHK from the indigenous peoples of South Africa, Africa and the rest of the world, and that there should be a clear plan as to how the knowledge can be supported within a health care systems approach.

COVID-19 has exposed the racialized nature of food systems, but also potentially grants opportunities to build anew. Maywa Montenegro explores a series of breakdowns, from fractured supply chains to uncontrolled infection among essential food workers to Black, Brown, and Indigenous communities scythed through by the virus along old grooves of race-class oppression. She traces the likely origins of COVID-19 to spillover sites forged by agroindustrial expansion into forested regions where pathogens spring free and infect humans. Industrial animal agriculture drives these ecological changes that incubate future outbreaks. Pandemics have their roots in the violent separation of communities from their territories, seeds, knowledge and wealth. Racism enables such theft as fundamental to capitalist expansion. To tackle pandemics and food injustices, Montenegro calls for an abolitionist agroecology. No anti-capitalist alternative can ignore the racism that is central to the transnational industrial food system. Scholars including Angela Davis, Ruth Wilson Gilmore, and Mariame Kaba have argued that although abolition is frequently seen as an oppositional strategy - to eradicate, for example, prisons and police - abolition is equally propositional. An abolitionist agroecology cracks open multiple possibilities that respond to the exigencies of a pandemic planet - there is no 'normal' to which we can safely return.
