Myocardial Ischemia and Lipid Metabolism by R. Ferrari Pdf

Over the past years, the thrust of research in cardiology has been toward an understanding of the engineering of the heart as a pump that transports blood to the various organs of the body. More recently, the fields of biochemistry and biophysics have come to influence heart research. The modern cardiologist can no longer pretend to understand, for example, what is happening to the patient with myocardial infarction or ischemia without understanding the principles of molecular biology. The structure and function of the heart are therefore central themes of cardiological research and practice, which incorporate knowledge and discoveries from diverse disciplines. The importance of lipid metabolism in the myocardium has become clearly understood. In the well-oxygenated heart, fatty acids are the preferred substrates. The fact that the heart derives most of its energy from the oxidation of lipids, which represent the larg est energy store of the body, is logical for an organ that must work throughout our lifetime. There are, however, several lines of evidence that during ischemia, lipids may. be harmful to the heart. High levels of free fatty acids in the serum have been suggested to play a role in causing life-threatening arryhthmias and damage in the ischemic heart. The molecular basis for these effects remains poorly understood, and several possible mechanisms for these harm ful effects have been suggested.

Lipid metabolism in the normoxic and ischaemic heart by H. Stam,G.J. van der Vusse Pdf

Scientific progress with respect to the important role of various lipids in myocardial function and the dearrangements of lipid metabolism underlying ischaemic heart disease(s) has been con siderable in recent years. In 1986 alone, an overall number ofl 044 full papers covering the topics "Lipids-Heart-Heart Disease", have been published in a variety of biochemistry, physiology, pharmacology and cardiology journals (source: Index Medicus). They have broadened our in sight into the molecular basis of myocardial lipidology and the lipido-chemical basis of car diology in health and disease, and have narrowed the gap between promising pharmacological intervention in experimental animals and clinical treatment of patients suffering from an ischaemic heart. Furthermore, they illustrate the fundamental significance of the product of the union between basic and clinical science. The rapid development of knowledge in the "Lipids-Heart-Heart Disease" triad prompted us to organize the International Symposium on Lipid Metabolism in the Normoxic and lschaemic Heart, held in Rotterdam (The Netherlands) on September 22 and 23, 1986. This meeting was a coproject of the Department of Biochemistry I (Medical Faculty, Erasmus University Rotter dam) and the Department of Physiology (Medical Faculty, University of Limburg) and was held under the auspices of the GerritJan Mulder Foundation (named after a famous Rotterdam physi cian and chemist who lived from 1802-1880) whom we thank for their co-operation. The present volume is a compendium of invited papers and contributions selected from the posters presented during the symposium.

Lipid Metabolism in Normoxic and Ischemic Heart by Ger J. van der Vusse Pdf

During recent decades, bewildering progress has occurred in the field of Molecular and Cellular Biochemistry. Progress has been extraordinarily rapid primarily because of the challenge for finding solutions to a wide variety of diseases and the availability of new techniques for monitoring biochemical processes. This has resulted in a voluminous and complex literature in the field of biochemical medicine so that there is a clear need for the synthesis and analysis of the continuing expansion of valuable data. It was thus considered appropriate to initiate a new series of monographs, each dedicated to a specialized area of investigation, encompassing molecular and cellular processes in health and disease. Most of the biochemical scientists have devoted their energies in understanding the fundamentals of biochemistry and indeed impressive advances have been made in the past. However, the full potential for explanation has been hampered by the concept of universality of biochemical reactions occurring in the cell. In view of the fact that each organ in the body performs a distinct function, it is now beginning to be realized that each cell type is unique in its need to survive and perform its specific function. Accordingly, the aspect of individualty is receiving increased attention for revealing new avenues in the study of pathophysiology of cellular abnormalities.

Myocardial Ischemia by Dennis V. P. Cokkinos,C. Pantos,Gerd Heusch,H. Taegtmeyer Pdf

Effective new treatments of heart disease are based on a refined understanding of cellular function and the heart's response to environmental stresses. Not surprisingly therefore, the field of experimental cardiology has experienced a phase of rapid expo nential growth during the last decade. The acquisition of new knowledge has been so fast that textbooks of cardiology or textbooks of cardiovascular physiology are often hard-pressed to keep up with the most important conceptual advances. Witness the explosive increase in knowledge about signaling pathways of cardiac growth, transcrip tional regulation of cardiac metabolism, hormonal signaling, and the complex responses of the heart to ischemia, reperfusion, or ischemic preconditioning. This book is meant to bridge the gap between original literature and textbook reviews. It brings together inves tigators of various backgrounds who share their expertise in the biology of myocardial ischemia. Each chapter is a self-contained mini-review, but it will soon become apparent to the reader that there is also a common thread: Molecular and cellular cardiology has never been more exciting than now, but ever more exciting times are yet to come. Cardiac Energy Metabolism in Health and Disease by Gary D. Lopaschuk,Naranjan S. Dhalla Pdf

The heart has a very high energy demand but very little energy reserves. In order to sustain contractile function, the heart has to continually produce a large amount of ATP. The heart utilizes free fatty acids mainly and carbohydrates to some extent as substrates for making energy and any change in this energy supply can seriously compromise cardiac function. It has emerged that alterations in cardiac energy metabolism are a major contributor to the development of a number of different forms of heart disease. It is also now known that optimizing energy metabolism in the heart is a viable and important approach to treating various forms of heart disease. Cardiac Energy Metabolism in Health and Disease describes the research advances that have been made in understanding what controls cardiac energy metabolism at molecular, transcriptional and physiological levels. It also describes how alterations in energy metabolism contribute to the development of heart dysfunction and how optimization of energy metabolism can be used to treat heart disease. The topics covered include a discussion of the effects of myocardial ischemia, diabetes, obesity, hypertrophy, heart failure, and genetic disorders of mitochondrial oxidative metabolism on cardiac energetics. The treatment of heart disease by optimizing energy metabolism is also discussed, which includes increasing overall energy production as well as increasing the efficiency of energy production and switching energy substrate preference of the heart. This book will be a valuable source of information to graduate students, postdoctoral fellows, and investigators in the field of experimental cardiology as well as biochemists, physiologists, pharmacologists, cardiologists, cardiovascular surgeons and other health professionals.

Myocardial Ischemia and Preconditioning by Naranjan S. Dhalla,Nobuakira Takeda,Manjeet Singh,Anton Lukas Pdf

Current evidence suggests that the ischemic preconditioning response is a multi-factorial process consisting of an initial early trigger, an intermediate mediator, and an end effector. Each of these steps in is now its own intense area of investigation. The need to render the heart ischemic for a brief period to invoke the preconditioning response is currently the major factor limiting clinical application of this powerful cardioprotective strategy. Recent research efforts have utilized brief exposures to pharmacological agents, in lieu of a brief preconditioning ischemia, to trigger/mimic the ischemic preconditioning-induced response. The World Heart Congress held in Winnipeg in July 2001 provided a forum for the presentation of new insights into the basic mechanisms of ischemia and reperfusion injury, as well as novel strategies to protect the heart from cell death, ventricular arrhythmias, and contractile dysfunction. Many pioneers in the fields of ischemia-reperfusion injury and preconditioning-induced protection presented there and the chapters in this book represent selected papers from these symposia.

Drugs Affecting Lipid Metabolism by Antonio M. Gotto Pdf

Even a brief scan of the table of contents of the present volume is enough to disclose the diversity of research interests and opinions in the field of lipidology. It is precisely this diversity that is the strength of our field and that was showcased by the XII International Symposium of DRUGS AFFECTING LIPID METABOLISM (DALM). The papers published here from these proceedings may be divided into three categories: those that define-and refine---our understanding ofthe clinical benefit of aggressive lipid management, those that develop our knowledge of ris!. assessment, and those that discuss the genetic, bio~hemical, and biophysical mechanisms underlying the pathology of coronary heart disease. On the clinical front, further analysis of the results of the Scandinavian Simvastatin Survival Study (4S) has indicated the cost-effectiveness of therapy in patients with established coronary heart disease. The West of Scotland Coronary Prevention Study (WOSCOPS), whose methodology was described at the DALM XII symposium, has demonstrated in a mostly primary-prevention population what 4S demonstrated for secondary prevention the year before: aggressive lipid-regulating therapy reduces coronary heart disease morbidity and mortality rates without concurrently increasing mortality from noncardiovascular causes. In the future, important considerations will be to develop protocols that maximize benefit in groups underrepresented in traditional clinical research for example, women and the elderly-and to improve compliance to existing treatment regimens. Furthermore, antioxidant, omega-3 fatty acid, and gene therapies warrant further investigation.
