But hyperthermia, as with any other technologically based medical procedures, requires an extensive develop ment of sophisticated instrumentation and techniques to offer routine clinical benefit. We probably erred in starting clinical trials so soon. We had hoped that by showing the clinical benefits on some superficial tumors quickly, financial support would be stimulated for the required technolog ical developments. Unfortunately, treating superficial disease adequately was more difficult than we had supposed and regional treatments were less successful than we had wished. The physical reasons are clear and were apparent from the beginning, although in our enthusiasm we ignored them. Circumstances are different now. We have to treat a wide range of tumors in various sites, but the systems and techniques required are only available in a few laboratories and clinics where they still are undergoing refinement.

Over the past several years, interest and activity regarding the use of heat to treat human malignancies has grown very rapidly. Laboratory studies with cells and animals have established the potential of hyperthermia, used alone, or in combination with radiation or drugs, for improvement of the therapeutic ratio in cancer therapy. A growing body of clinical data suggests that hyperthermia, employed as an adjuvant and perhaps, as a primary therapeutic modality, can often effect rapid and substantial tumor regression while causing only relatively modest changes in adjacent normal tissues. Given the clinical observations to date and the rationale provided by biological and physiological laboratory investigations, the current, prodigious growth of clinical utilization of hyperthermia in surgical, medical and especially radiation oncology is not difficult to comprehend. However, the development of clinical hyperthermia as a safe, effective and quantitative cancer modality will depend critically on the extent to which the physics and physiology of local, regional and whole body heating of human tissue are understood and properly incorporated into the planning and administration of thermotherapy. Whatever its promise, ultimately, the value of hyperthermia as a clinical tool will be governed, and perhaps limited by the physical aspects of power deposition, heat transfer and thermometry in vivo. The present challenge in the treatment of tumors is to reduce the number of patients that still die as a result of primary tumors. Today, the percentage of such deaths remains high at 30%, even when all the common therapeutic methods, namely surgery, radiotherapy, and chemotherapy, are applied. In order to reduce this percentage, new types of radiation sources with a higher linear energy transfer have been intro duced, such as neutrons and pions. Fractionation patterns have been modified and radiosensitizers have been applied to in crease biological efficiency. Studies of the combined application of chemotherapy and radiotherapy have been made to find the best therapeutic effect. In the early 1970s biological findings confirmed the effect of hyperthermia on tumor cells. The first clinical studies on hyper thermia treatment demonstrated that it resulted in better local tumor control. Further application of this treatment modality showed that hyperthermia should be used in addition to radio therapy and chemotherapy. Despite these encouraging results, hyperthermia has not been introduced into common clinical use, due primarily to technical problems. There are a number of methods of transferring heat into tumors; however, with regard to physical conditions, an op timum method has not yet been found. One of the reasons is that up to now we have had no reliable method of obtaining thermal mapping of all parts of the human body. Such measurements are required not only for dosimetric purposes but also for the regula tion of a hyperthermic system.
