The increasing prevalence of preterm birth in the United States is a complex public health problem that requires multifaceted solutions. Preterm birth is a cluster of problems with a set of overlapping factors of influence. Its causes may include individual-level behavioral and psychosocial factors, sociodemographic and neighborhood characteristics, environmental exposure, medical conditions, infertility treatments, and biological factors. Many of these factors co-occur, particularly in those who are socioeconomically disadvantaged or who are members of racial and ethnic minority groups. While advances in perinatal and neonatal care have improved survival for preterm infants, those infants who do survive have a greater risk than infants born at term for developmental disabilities, health problems, and poor growth. Understanding the complexity of interactions underlying preterm birth will be needed if further gains in outcomes are expected. The Institute of Medicine's Roundtable on Environmental Health Sciences, Research, and Medicine sponsored a workshop to understand the biological mechanism of normal labor and delivery, and how environmental influences, as broadly defined, can interact with the processes of normal pregnancy to result in preterm birth. The "Development of Attachment and Affiliative Systems" was selected as the topic for a three-day workshop held at Estes Park, Colorado, in May, 1980. The papers which resulted from this effort not only reflect a recent intensity of research in this area, but also highlight a mounting need for ask ing questions across disciplines and for integrating theories. The sponsor of the workshop was the Developmental Psychobiology Research Group (DPRG) of the Department of Psychiatry, University of Colorado Medical School, a group which itself is interdisciplinary and which has met regularly since 1969 to criticize research, ask questions, and discuss findings. In 1974, the Group was awarded an endowment fund by the Grant Foundation after a request for a proposal initiated by Philip Sapir and Douglas Bond. The aims of this fund are to facilitate the research of young investigators, to encourage new research, and to provide seed money for collaborative ventures. Much of what is reported here results from that support. Thus, happily, not only are the contributions timely by virtue of converging on an important topic, but they also commemorate more than five years of Grant Foundation support. Once the topic was chosen, a small number of guests were invited to participate. The papers of Timiras, Sackett, Konner, and Lamb represent dif fering perspectives from neurobiology, primatology, cultural anthropology, and social psychology.

"Although the benefits of psychological consultation in the pediatric setting are well established, a gap often exists between the demand for these services and funding. We have embarked on our longstanding goal to develop a group-based intervention model for parents of premature infants, adapting our manual of individual trauma-focused cognitive behavioral therapy to help reduce feelings of parental isolation. This book describes a more global approach to psychological consultation in the NICU integrating interventions that begin prior to the infant's conception and extend well beyond the NICU hospitalization. Chapter 1 provides a context and review of the medical aspects of the NICU environment and the neurodevelopmental consequences of prematurity. In Chapter 2 reviews the common psychological reactions of mothers of premature infants, including specific risk factors associated with maternal psychological distress. It also discusses the relationship between parental posttraumatic stress and infant outcomes as it relates to such issues as breastfeeding, maternal-infant interaction, attachment, and infant development. Chapter 3 describes the form and prevalence of symptoms of paternal psychological distress and outline a curriculum for a group-based intervention specifically designed to address fathers' concerns. Chapter 4 addresses developmental care interventions that overlap with interventions more narrowly focused on parental psychological distress. Chapters 5 and 6 describe our intervention model in both the individualand group therapy formats. Chapter 7 addresses vulnerable child syndrome, which is associated with adverse developmental outcomes in children as well as overutilization of health care resources. Application of the trauma model to the concept provides a framework to understand how parental behavior is altered in the context of trauma. Finally, Chapter 8 discusses how to implement a psychological intervention program in the NICU that includes screening the parents of premature infants for symptoms of psychological distress"--
