This publication summarizes the evaluation of Phase V (2009-2013) of the WHO European Healthy Cities Network. The evaluation process was designed in collaboration with city representatives, academic institutions and public health experts. It adopted a realist synthesis approach, being responsive to the unique social, cultural, political, health and epidemiological circumstances in the 99 cities in the WHO European Healthy Cities Network and 20 accredited national networks. The evaluation findings are rooted in the enduring healthy city values such as equity, governance, partnership, participation and sustainability. Considering also the core Phase V themes, this publication focuses on policy and governance, healthy urban environments and design, caring and supportive environments, healthy and active living, national network performance and effects on health and equity. The evaluation finds good progress among cities and networks but differing in scale and quality. In recent years, a considerable number of countries have developed and implemented strategies aiming at reducing inequalities in health. However, knowledge of effective policies or strategies to reduce socioeconomic inequalities in health is still very fragmented. The aim of this book is to describe and compare different European health equity strategies and their potential successes. The main part of the material comes from a comparative study of national public health strategies for equity in health with the following participating countries: Denmark, Finland, England, Italy, Netherlands, Norway, Spain and Sweden. However, there is considerable lack of relevant data to enable comprehensive comparisons and analyses of successes and problems with the implementation and monitoring of health equity policies. We hope the book will stimulate continued efforts to harmonise relevant data collections and improve collaborative analyses aiming at shaping an evidence base for policy decisions about the most effective ways to promote equity in health.

Countries in Europe have long recognized that good health care, though essential, is not in itself sufficient to improve health or to reduce the increasing gaps in health status between the rich and the poor. In 1984, together with WHO, they adopted and many attempted actively to implement, what has become known as the health for all policy. This called for a radical shift from health services planning to an approach based on setting objectives and targets for health, requiring partnerships with industry, agriculture and commerce and settings such as workplaces and schools. Evidence indicates that actions within four main themes (early child development fair employment and decent work social protection and the living environment) are likely to have the greatest impact on the social determinants of health and health inequities. A systematic search and analysis of recommendations and policy guidelines from intergovernmental organizations and international bodies identified practical policy options for action on social determinants within these four themes.
