The heart, however, is not only the target for the direct and indirect actions of a number of hormones and humoral factors, it is also an endocrine organ in the traditional sense, synthesizing and secreting into the circulation chemical factors that act at distant sites. In this treatise, Hormones and the Heart in Health and Disease, we interpret "endocrinology" broadly and consider traditional hormones as well as autocoids that are secreted by the heart or that act on it. The discovery of atrial natriuretic peptide (ANP; atrial natriuretic factor, ANF) in the 1980s indicated that the heart does indeed function as an endocrine organ in the classic sense. ANP (Chapter I) is synthesized in the heart and secreted into the circulation for actions on the kidney, where it is a potent natriuretic agent, and on the vasculature, where it causes vasodilation. The unique biology of cardiac fibroblasts and related cells, such as cardiac myofibroblasts and valvular interstitial cells, distinguish them from other fibroblastic cells, a concept that is only beginning to be widely appreciated. Further, the natural signals that stimulate and inhibit cardiac fibrosis within these cells are not well understood. This volume compiles articles that address the molecular mechanisms that control the synthesis and secretion of the cardiac ECM. The book showcases chapters that highlight discussion of role of Transforming Growth Factor β (TGFβ), an important fibrogenic cytokine and its downstream effectors SMAD in many cardiac diseases. Further, the contributions highlight information to discuss endogenous inhibitors of cardiac fibrosis, as well as advances in tissue engineering specific to matrix in the heart. Finally, discussions of unifying mechanisms of matrix remodeling in valves and myocardium are presented. The mechanisms involved in the stimulation of cardiac fibrosis are not fully understood. In most cases the marginal attenuation of cardiac fibrosis as a result of a given therapy is a beneficial side-effect linked to other primary effects on other cells, especially cardiomyocytes. Very few drugs or agents are known to affect the function and dysfunction of cardiac fibroblasts and myofibroblasts alone. The book helps to translate the information gathered within to allow us to alter the course of fibrogenic events that are typical of cardiac fibrosis, and thereby reduce their burden on the patient and on society itself.

Platelet-activating factor (PAF) is the commonly used name for a group of potent biologically active, ether-linked phospholipids, the alkylacetylglycerophospho cholines. Following the identification of the chemical structure of P AF in 1979, much progress has been made in our understanding of these phospholipid medi ators. This is particularly true from the points of view of their molecular structural requirements for expression of biological activity, their biosynthesis and cata bolism, their diverse range of biological activities, evidence for their specific recep tors at the cell surface, the development of specific antagonists for use in receptor studies and for testing as potential anti-PAF drugs, and implications about their involvement in health and disease. Chapters in this book cover the current status of these general areas of PAF research. Although an enormous amount of information has been published about this interesting new type of phospholipid mediators, questions about their mechanism of action, regulatory controls, and the precise role and relative importance of P AF in specific diseases and physiological functions remain to be answered. Since the 1979 discovery, the PAF field has brought together scientists from a variety of different biomedical disciplines. The diversity of scientists now in PAF research is very apparent from the number of P AF publications appearing in a wide array of scientific journals, ranging from molecular biology to clinical medicine.
