Because of several valid (and some invalid) reasons, the research field of tumor immunology has been declining in popularity. The Simplistic dogmas, articles of faith, and theories of the late 1960s and early 1970s on the immuno logical mechanisms of the host-tumor interrelationships have frequently been refuted by some of the new developments in cancer biology, cancer biochem istry, and immunology. Furthermore, some of the conventional assays used to monitor "tumor-host immune relations" did not always reflect the host's true clinical situation or his prognosis. Several approaches to immunological interven tion were less successful than expected. In addition, the concept of "immune surveillance," which was basic to many researchers in the field of cancer im munology, seemed to fall apart. Much of the criticism was based on results from solid, well-performed, and well-controlled experiments, but there was also un just criticism based on ill-conceived and badly performed studies, and on misin terpretations of experimental data. There are many misconceptions about the tumor-host relationship. It is very often assumed that tumor immunity, as expressed systemically, is truly reflected at the tumor site. Several studies reported in this volume and elsewhere indicate that such is not always the case. Certain immune effectors may be selectively prevented from reaching the tumor site or the close vicinity of the tumor cells because of mechanical or chemical barriers, whereas others may be selectively attracted to the site by chemotaxis or other mechanisms.

The immune system can deal effectively with the majority of viruses and bacteria, less effectively with parasites, and very poorly with cancer. Why is this so? Why are McFarlane Burnet's and Lewis Thomas' predictions that the immune system is in volved in ridding the body of cancer cells, encapsulated in the catchy phrase "immunologic surveillance," so difficult to experi mentally establish? Cancer differs from infectious agents in being derived from the host. Hence, it has been postulated that cancer cells lack anti gens that the immune system can recognize. They are not "im munogenic. " However, this argument is seriously weakened by the existence of numerous human autoimmune diseases, in which the immune system effectively recognizes and attacks a va riety of self tissues. Thus, the potential clearly exists for recogni tion of the surfaces of tumor cells. Professor Naor and his colleagues have written a book that explores another possible reason: cancer cells are recognized by the immune system-but is it possible that the consequence of recognition is inhibition of the immune system-by suppressor T cells or macrophages? The evolution of the malignant state may only occur in individuals who develop this suppression. This book reviews the evidence that suppressor cells, poorly characterized and difficult to study, may be of fundamental im portance in cancer. In fact, our incapacity to understand the na ture of suppressor cells and their mode of action is one of the ma jor problems in immunology research today.

There should be specificity, so that tumour cells alone are destroyed whilst normal tissues are unaffected. Provided the tumour is uni form and all of the cells have appropriate antigens, every malignant cell should be destroyed and even distant metastases dealt with. So far these speculative advantages are unfulfilled and the initial optimism that surrounded im munotherapy has not been sustained. Acceptance of the precepts of tumour im munology continues but these disappointing observations had led to increasing scrutiny of certain aspects. The purpose of this chapter is to review the prin ciples which underly tumour immunology and immunotherapy, so that the more detailed studies that follow can be considered in perspective. TUMOUR ANTIGENS (Chapter 2) For a tumour to initiate an immunological response, it must possess distinctive antigens. Much of the early work in tumour immunity was confused because it was not appreciated that tumours, like other tissues, exhibit transplan tation antigens. Only when syngeneic tumours are used can tumour antigens alone be studied and it was the introduction of inbred mouse strains which allowed Foley in 1953' to produce the first evidence for specific an tigenicity of experimental tumours. Demonstration of these antigens requires that pretreatment with syngeneic tumour will influence the growth of a sub sequent challenge with the same neoplastic cells.

This Scientific Publication reviews the information on cancer sites and mechanistic events for the more than 100 agents classified in Group 1 (carcinogenic to humans) by the IARC Monographs Program. This category of agents is diverse and includes chemicals and chemical mixtures; occupations; metals, dusts, and fibres; radiation; viruses and other biological agents; personal habits; and pharmaceuticals. For the Group 1 agents, there were cross-cutting questions about the relevance to humans of certain cancer sites or mechanistic pathways in animals. This publication is based on a systematic identification and comparison of the cancer sites observed in humans and those observed in experimental animals, and a compilation of mechanistic events for agents known to cause cancer in humans. Relevant information was analyzed on all the agents classified in Group 1 in Monographs up to and including Volume 109, most of which are reviewed in Volume 100A-F. A database of tumor sites seen in humans and animals was used to examine the degree of concordance by use of an anatomically based tumor classification scheme. The analysis of mechanistic aspects of the IARC Group 1 agents focused on 10 key characteristics of human carcinogens developed during the course of this work. Genotoxicity was the most prevalent mechanistic characteristic, consistent with the process of carcinogenesis necessarily involving genomic changes. The IARC concordance database represents a useful source of information for comparing animal and human data with respect to the tumors caused in different species. The results of the mechanistic analysis can provide a basis for future efforts to categorize mechanistic data for carcinogens through a systematic review process. These reviews and analyses were discussed during a two-part Workshop on Tumour Site Concordance and Mechanisms of Carcinogenesis convened by IARC. This Scientific Publication is the report of that Workshop and of subsequent work by the participants, both individually and collectively. This publication also presents a statement of consensus among the Workshop participants, which summarizes the main findings and their implications for human cancer risk assessment.
