Cardioprotection by Derek Hausenloy,Derek Yellon Pdf

Coronary heart disease (CHD) is the leading cause of death worldwide with 3.8 million men and 3.4 million women dying each year. Cardioprotection refers to the prevention of CHD and the clinical improvement in patients suffering from cardiovascular problems. This book focuses on the role of cardioprotection in surgery and the use of pharmacological therapies such as ACE-inhibitors, statins and beta-blockers in order to reduce the myocardial injury sustained by the patient and the significant risk of morbidity and mortality. It includes new cardioprotective strategies aimed at improving the clinical outcomes of patients in these settings, as well as current well-established methods for reducing myocardial injury in acute coronary syndrome patients.

Development of a Diagnostic Device to Predict Clinically Significant Inflammation Associated with Cardiac Surgery by Mohammed Yahya Majrashi Pdf

Background: Cardiopulmonary bypass deployment is known to cause an inflammatory response during open heart surgery. Inflammation involves the activation of different cascades such as coagulation, the complement system and cytokines. Although the immune system is the body's key defense mechanism against external assault, it can be injurious to the patient when the immune system is over-expressed, particularly in cohort of patients that experience a heightened and uncontrolled response. This exaggerated response results in autoimmune injury and may lead to poor post-operative outcomes, such as systemic inflammatory response syndrome and multi-organ failure. Aim of the work: This project was aimed to develop a predictive screening technology that enables clinicians to specify patients who may be at risk prior to open heart surgery. This project suggests a new approach to identifying these patients, through the development of a novel technology that will measure the extent of the inflammatory response following blood contact with extracorporeal systems. This work focuses on cardiopulmonary bypass, but the technology may also extend to other treatments and interventional modalities in which tissues, in particular blood, comes into contact with foreign surfaces.

Atrial Fibrillation after Cardiac Surgery by Jonathan S. Steinberg Pdf

Cardiac surgery is performed on hundreds of thousands of patients a year, and can have an important beneficial impact on the outcomes of patients with coronary and valvular heart diseases. Despite the favorable recovery of most patients, some will have their post-operative period interrupted by the development of atrial fibrillation, with a host of potential complications including stroke. High risk subgroups may develop atrial fibrillation in more than half of cases, and often despite aggressive prophylactic measures. Treatment of atrial fibrillation and its aftermath can also add days to the hospital stay of the cardiac surgical patient. In an era of aggressive cost cutting and optimization of utilization of health care resources, the financial impact of this arrhythmic complication may be enormous. Experimental studies have led to a greater understanding of the mechanism of atrial fibrillation and potential precipitating factors in the cardiac surgical patient. Prophylactic efforts with beta-blockers, antiarrhythmic drugs and atrial pacing are being used, or are being investigated in clinical trials. New methods of achieving prompt cardioversion with minimal disruption of patient care, and prevention of the thromboembolic complications of atrial fibrillation, are also important therapeutic initiatives. This text is designed to aid health care professionals in the treatment of their patients in the recovery period after cardiac surgery, and to instigate additional research efforts to limit the occurrence of, and the complications following, this tenacious postoperative arrhythmia.

Peripheral Vascular Surgery by Martin Birnstingl Pdf

Peripheral Vascular Surgery aims to provide an updated and authoritative introduction to peripheral vascular surgery. Most arterial and venous diseases appear to be mainly the result, directly or indirectly, of mural deposition of solid material within a rapidly moving stream of blood. The vascular surgeon must therefore have a working knowledge of blood platelet interactions and probable mechanisms of thrombosis. He must also understand both the value and the limitations of present methods of blood flow measurement. This book provides an updated description of these complexities and presents essential basic knowledge that will prove valuable to surgeons and researchers working in this field. This text will be useful for post graduate surgeons, as well as for those in need of a reference useful for doctors in related fields such as general medicine and cardio-thoracic and orthopedic surgery.

Coronary Microvascular Dysfunction by Filippo Crea,Gaetano A. Lanza,Paolo G. Camici Pdf

In the past two decades a number of studies have shown that abnormalities in the function and structure of coronary microcirculation can be detected in several cardiovascular diseases. On the basis of the clinical setting in which it occurs, coronary microvascular dysfunction (CMD) can be classified into four types: CMD in the absence of any other cardiac disease; CMD in myocardial diseases; CMD in obstructive epicardial coronary artery disease; and iatrogenic CMD. In some instances CMD represents an epiphenomenon, whereas in others it represents an important marker of risk or may contribute to the pathogenesis of myocardial ischemia, thus becoming a possible therapeutic target. This book provides an update on coronary physiology and a systematic assessment of microvascular abnormalities in cardiovascular diseases, in the hope that it will assist clinicians in prevention, detection and management of CMD in their everyday activity.
