Laboratory based scientists now predominate over those who work as clinical specialists in VIi rheumatology, immunology, oncology and paediatrics. However, there continue to be major clinical objectives since large sections are concerned with major causes of death like ATP depletion, cancer and now AIDS; the laboratory work is providing clinical solutions.

th This volume contains articles presented at the X International Symposium on Purines and Pyrimidines in Man, held on May 14 19, 2000 in Tel Aviv, Israel. The first symposium in this series took place in Tel Aviv in 1973. Since then, the symposium has been held every three years in different parts of the world, including Europe, USA and Japan. The participants, in this series of symposia, are characterised by a wide interest in the various aspects of purines and pyrimidines in man, which include biochemistry, genetics, pharmacology, physiology, clinics, etc. Presentations in the symposia include clarification of metabolic pathways, characterisation of enzyme structure and kinetics and discoveries of new inborn errors of metabolism and suggestions for new therapeutic approaches for these inborn errors. Early in the 1950s, I was engaged in clinical practice treating various rheumatic diseases as an orthopedicist, and found that a substantial percentage of our patients had symptoms apparently compatible with gout. During the 1960s, the number of these gouty patients increased, and in the 1970s, research on the pathogenesis of gout was performed on the basis of approximately 2,000 cases of this disease, together with precise epidemiological studies concerning gouty and hyperuricemic individuals. Data derived from the two kinds of study had greatly changed the notion that gout was a rare disease among Japanese. My clinical studies have been succeeded by research at the molecular level on various purine metabolic abnormalities, including not only gout but also other diseases with various symptoms of wide clinical spectra.

In September, 1990, a group of 160 scientists from 19 countries and 21 of the United States met at the Red Lion Inn in Rohnert Park, Sonoma County, California. The purpose of this meeting was to share new information from recent research on the Aspartic Proteinases. This book is a compilation of the information transferred in that forum. The Aspartic Proteinases include all those enzymes from the "fourth" class of proteolytic enzymes, the first three being the Serine, Cysteine and Metalloproteinases. Of course, all the scientists in attendance at the Sonoma Aspartic Proteinase Conference would agree that our current level of understanding of the structure and function of the Aspartic Proteinase class of enzymes is clearly first class. The reasons for this require a bit of historical perspective. The group of scientists who are engaged in study of this family of enzymes first met as a separate entity in 1976, in Norman, Oklahoma, at a meeting organized by Jordan Tang of the Oklahoma Medical Research Foundation. This was an exciting time, as the first crystal structures of some of these enzymes were described by Blundell, James and Davies. During that conference, the relationship between the two halves of the mammalian and fungal enzymes was recognized and this has provided a structural foundation for analysis of the retroviral enzymes, which came later. A book was published by Plenum Press documenting l this conference, and the current book is an update to that important work.

Hypertension is recognized to be one of the major risk factors for the development of peripheral vascular disease. The last decade has witnessed several major advances in therapy for hypertension, including the development of angiotensin-converting enzyme inhibitors and calcium channel blockers. These compounds have greatly improved the ability to control blood pressure and to reduce the impact of this risk factor on morbidity and mortality. In spite of these advances, cardiovascular disease remains a major health problem in most modern industrialized countries with related deaths exceeding those from all other causes combined. In contrast to these advances in therapy, our understanding of the basic mechanisms responsible for the pathogenesis of hypertension remains incomplete. Recent studies have produced new insights into the nature of the regulation of muscle contraction in both heart and blood vessels as well as the changes in muscle function that occur in hypertension. However, the effects of antihypertensive therapy, both in terms of restoring normal function and in producing reversal of hypertension-associated changes, has not been as thoroughly studied, especially in the vasculature. Studies in the heart suggest that the efficacy of different therapeutic agents in restoring normal function and reversing hypertensive changes vary substantially with the mechanism of action of the therapeutic agent. It has also been recently determined that some therapeutic agents produce adverse effects on plasma lipid profiles, which could lead to the secondary acceleration of the atherosclerotic process, while at the same time normalizing blood pressure.

The Third International Symposium on Excitation-Contraction Coupling in Skeletal, Cardiac, and Smooth Muscle, organized by George Frank, C. Paul Bianchi, and Henk E. DJ. ter Keurs, was held in Banff Centre, Banff, Alberta, Canada during June 26 to June 30, 1991. The theme of these symposia has been to recognize the similarities and dissimilarities of excitation-contraction coupling in skeletal, cardiac, and smooth muscle. Cross fertilization of concepts of excitation-contraction coupling in these three types of muscle has occurred since the early studies in the late fifties and early sixties on skeletal muscle. Investigators in each field meet only at specialized symposia which exclude investigators in the other fields. The purpose of the symposia has been to bring together international investigators studying excitation-contraction coupling in skeletal, cardiac, and smooth muscle so that we may learn from each other and hence provide a more global concept of excitation-contraction. The Third International Symposia has accomplished its objective as we recognize that calcium channels of the sarcolemma and the sarcoplasmic reticulum play key essential roles in excitation-contraction coupling in all three types of muscles. In skeletal muscle the recognition that E-C coupling consists of two parallel mechanisms, one dependent upon a dihydropyridine voltage-sensitive sensors coupled to calcium release from the terminal cisternae via the ryanodine sensitive channel in the foot structure of the triad.

In the course of history, humans have attempted to interrupt the physiological and psychological bond formed between a nursing mother and her child by substituting breastfeeding with artificial formulas. A growing body of evidence indicates that breast milk, quite apart from its unsurpassed nutritive value, contains a large number of substances that protect the offspring from common infectious agents and allergens and promote the maturation of the gastrointestinal tract and the immune system. In addition to well described milk antibodies and soluble mediators of innate immunity, milk cells and pluripotent secreted factors - cytokines - are currently in the forefront of extensive research with respect to their importance in milk immunology. The purpose of this conference was to critically evaluate the current state of our knowledge concerning the protective role of immune agents found in milk, to provide up-to-date information of milk factors with respect to their role in the maturation of immunological defense systems in the neonate, and to reassess the importance of breastfeeding in the prevention of allergies in formula-fed infants. We hope that the work presented by international participants will prompt many new ideas and stimulate further research in this important area.
