This book explores the economic and broader societal rationale for using unmanned aerial vehicle (UAV) or “drone†? technologies as a complement to the current transport and logistics systems in several use cases in East Africa. The specific use cases examined include medical goods deliveries, food aid delivery, land mapping and risk assessment, agriculture, and transport and energy infrastructure inspection. Across these applications, the case for using UAVs is examined within the context of logistics objectives—total operating costs, speed, availability, and flexibility—as well as human, or societal, objectives. In the public health use case, as more low- and middle-income countries explore opportunities to improve efficiency and performance in their health supply chains and diagnostics networks, they face myriad choices about how best to use UAVs to improve product availability and public health outcomes and to reach the last mile. The high-level findings from this analysis are that, if examining commodity categories individually and looking exclusively at costs, delivery with UAVs in general is still more expensive for most categories. Although the cost is still higher, the most cost-effective use case examples include the transport of laboratory samples to selected destinations and delivery of life-saving items and blood. However, “layering†? several use cases can provide efficiencies and cost savings by allocating fixed costs across a greater number of flights and maximizing capacity and time utilization. From the perspective of public decision-makers, the cost effectiveness of UAVs cannot be analyzed without looking at the public health benefits, which may be substantial. Drone application in the other use cases examined in this book, such as mapping, risk assessment, and agriculture, is relatively more common than cargo drone operations, and the existing pilot initiatives in East Africa have delivered impressive results for speed and quality (precision). Food aid delivery by drones is still mostly at a planning, rather than implementation, stage. Drone applications are rapidly evolving, and several use cases could gain impact and scale over the coming years.

Armed conflict combined with prolonged drought has put about 20 million people at risk of starvation and death in Somalia, South Sudan, Yemen, and northern Nigeria. The international development and aid communities are caught between the enormity of the humanitarian crisis, which demands an estimated US$4.4 billion to address, and the lack of resources forthcoming from donors. Food crises, famine-like conditions, and famines recur with regularity in many developing countries (see Box 1 for definitions of terms). Although the current famines can be largely attributed to conflicts, chronic food insecurity also threatens several other African countries. For example, 6.7 million people were affected by Malawi’s largest food crisis in decades in 2016–2017, and the country remains vulnerable to weather extremes that could create food emergencies (World Bank 2017). In Kenya, food security has deteriorated since the end of 2016 and half of its 47 counties face food shortages (Chatterjee and Mengistu 2017). How do countries prepare to prevent shocks—natural and man-made—from generating food crises? What does it take to break the cycle of chronic food insecurity and build resilient food systems? According to estimates by the United Nations, more than 28 million people in east Africa need immediate food aid. Three countries in the region have emergency level food aid needs-Somalia, South Sudan, and Sudan. The devastating impact of the current famine isn't confined to the hardest hit drought areas. Uganda itself, struggling with the effects of drought in some areas, has had to contend with nearly 800,000 refugees from South Sudan. In 2011, the first U.N.-declared famine since the 1980s occurred in east Africa, affecting more than 10 million people. Six years ago, there was great attention given to that famine. The current famine, despite reaching crisis levels, has not garnered the attention that it deserves.

The Sudan illustrates the proverbial worst-case scenario in which urgent food needs have been denied, food has been used as a weapon, and outside assistance has been obstructed. The Challenges of Famine Relief focuses on the two famine emergencies in the Sudan in the 1980s—the great African drought-related famine of 1984-86 and the conflict-related famine that afflicted the southern Sudan in 1988-91. Francis Deng and Larry Minear analyze the historical and political setting and the response by Sudan authorities and the international community. The book outlines four problem areas exemplified in the response to each crisis: the external nature of famine relief, the relationship between relief activities and endemic problems, the coordination of such activities, and the ambivalence of the results. The authors identify the many difficulties inherent in providing emergency relief to populations caught in circumstances of life-threatening famine. They show how such famine emergencies reflect the most extreme breakdown of social order and present the most compelling imperatives for international action.
