Over the past years, the thrust of research in cardiology has been toward an understanding of the engineering of the heart as a pump that transports blood to the various organs of the body. More recently, the fields of biochemistry and biophysics have come to influence heart research. The modern cardiologist can no longer pretend to understand, for example, what is happening to the patient with myocardial infarction or ischemia without understanding the principles of molecular biology. There are, however, several lines of evidence that during ischemia, lipids may. be harmful to the heart. High levels of free fatty acids in the serum have been suggested to play a role in causing life-threatening arryhthmias and damage in the ischemic heart. The molecular basis for these effects remains poorly understood, and several possible mechanisms for these harm ful effects have been suggested.

The rapid development of knowledge in the "Lipids-Heart-Heart Disease" triad prompted us to organize the International Symposium on Lipid Metabolism in the Normoxic and lschaemic Heart, held in Rotterdam (The Netherlands) on September 22 and 23, 1986. Lipid peroxidation is an important cellular process which can lead to detrimental effects if it is not regulated efficiently. Lipid hydroperoxide is formed in an initial step of lipid peroxidation. Lipid hydroperoxide is also known as a potential source of singlet oxygen. Harmful aldehydes are formed when the lipid hydroperoxide is degraded. The formed aldehyde has high reactivity against thiol or amine moieties. Therefore, it could act as a signaling molecule, which might induce the changing of gears inside a cell. Recent studies have shown that lipid hydroperoxide or a slightly modified product of the lipid hydroperoxide reacts with biomolecules such as proteins and aminophospholipids, which leads to formation of amide-type adducts. Amide-type adducts could be one of markers for oxidative stress and could also be an important player in some diseases. In this book, the chemistry and biochemistry of lipid hydroperoxide along with their conjugates with biomolecules are described.

Heart failure continues to be a major public health problem in the United States with close to half a million new cases diagnosed each year. This is not to say that no progress has been made in the treatment of heart failure. While for many years treatment was confined to the management of the symptoms, in recent years with the advent of ACE inhibitor and ß blacker therapies, real improvements in cardiac function and life expectancy have been achieved (Volume 4B, Leier). On a more basic level, enormous advances have been made in describing many of the changes in structure and function of the heart and the parallel neurohumoral and circulatory adaptations that occur during the onset of failure. These advances have been made not only by using various animal models of heart failure, but also using fresh failing human heart tissue, which has become readily available for experimental investigation since the advent of cardiac transplantation. Understanding the significance of many of these changes that occur during the transition to failure and the role they play in the etiology of failure is, however, a much more difficult task. These are exciting times in heart failure research. It is as though many of the pieces of the jigsaw puzzle are available but the puzzle has yet to be assembled.
