During the recent transition between acute diseases caused by swarms of single planktonic bacteria, and chronic infections caused by bacteria growing in slime-enclosed biofilms, a general clinical consensus has emerged that pathologies with bacterial etiologies are frequently culture negative. Because biofilm infections now affect 17 million Americans per year (killing approximately 450,000), the suggestion that these common and lethal infections regularly go unnoticed by the only FDA-approved method for their detection and characterization is a matter of urgent concern. Biologically, we would expect that planktonic bacterial cells would colonize any new surface, including the surface of an agar plate, while the specialized sessile cells of a biofilm community would have no such proclivity. In the study of biofilm diseases ranging from otitis media to prostatitis, it was found that direct microscopy and DNA- and RNA-based molecular methods regularly document the presence of living bacteria in tissues and samples that are culture negative. The editors selected orthopedic biofilm infections as the subject of this book because these infections occur against a background of microbiological sterility in which modern molecular methods would be expected to find bacterial DNA, RNA-based microscopic methods would be expected to locate bacterial cells, and cultures would be negative. Moreover, in Orthopedics we find an already biofilm-adapted surgical group in which current strategies are based on the meticulous removal of compromised tissues, antibiotic options as based on high biofilm-killing local doses, and there are practical bedside strategies for dealing with biofilm infections. So here is where the new paradigm of biofilm infection meets the equally new paradigm of the culture negativity of biofilms, and this volume presents a conceptual synthesis that may soon combine the most effective molecular methods for the detection and identification of bacteria with a surgical discipline that is ready to help patients.

After the publication of the AO book Technique of Internal Fixation of Fractures (Miiller, Allgower and Willenegger, Springer-Verlag, 1965), the authors decided after considerable discussion amongst themselves and other members of the Swiss AO that the next edition would appear in three volumes. In 1969, the first volume was published (the English edition, Manual of Internal Fixation, appeared in 1970). This was a manual of surgical technique which discussed implants and instruments and in which the problems of internal fixation were presented schematically without radio logical illustrations. The second volume was to be a treatise on the biomechanical basis of internal fixation as elucidated by the work done in the laboratory for experi mental surgery in Davos. The third volume was planned as the culminating effort based upon the first two volumes, treating the problems of specific fractures and richly illustrated with clinical and radiological examples. It was also to discuss results of treatment, comparing the results obtained with the AO method with other methods. The second and third volumes were never published. The second edition of the AO Manual appeared in 1977. It dealt in greater detail with the problems discussed in the first edition, although it still lacked clinical exam ples and any discussion of indications for surgery. Like the first edition, it was trans lated into many languages and was well received. Finally, after 22 years, the much discussed and much needed third volume has appeared.

Care for patients who have suffered major trauma, for example following a road accident or a fall, has not significantly improved in the last 20 years despite numerous reports identifying poor practice, and services are not being delivered efficiently or effectively. Survival rates vary significantly, with a range from five unexpected survivors to eight unexpected deaths per 100 trauma patients, reflecting the variable quality of care. 450 to 600 lives could be saved each year in England if major trauma care was managed more effectively. For best outcomes care should be led by consultants experienced in major trauma; but major trauma is most likely to occur at night and at weekends, when consultants are not normally in the emergency department. Major trauma care is not coordinated and there are no formal arrangements for taking patients directly for specialist treatment or transferring them between hospitals. A significant number of patients that need a scan CT do not receive one. Not enough patients who need a critical care bed are given one. Access to rehabilitation services varies and patients are not always receiving the care that they need. The estimated annual lost economic output from deaths and serious injuries from major trauma is between £3.3 billion and £3.7 billion. Only 60 per cent of hospitals delivering major trauma care contribute to the Trauma Audit and Research Network (TARN). The performance of the 40 per cent of hospitals that do not submit data to TARN cannot be measured.
