Although distant metastases are the most dreaded situation in the evolution of cancer of every organ, the medical literature has surprisingly given little attention to the anatomical relationship between the primary tumor and metastasic sites. Only risk factors, treatment possibilities, and survival results are extensively examined. Stimulated by the occurrence in his practice of some puzzling and unexpected metastases, the author reviewed more than 12,000 references. He looked for anatomical relationships highlighting the relation between the location of the primary tumors and the particular patterns of metastasis observed. It would seem that the `pathways and flows' are apparently a more decisive factor in the implantation of the metastases than the `seed and soil' properties of the cancer cells and the metastatic site. Aided by his colleague Dr. T. Geukens, M.D., the author includes original anatomical drawings, illustrating the sometimes unexpected pathways the cancer cells follow in order to reach the organs where they will become lodged and give rise to metastatic tumors. The subject has apparently not been exhausted in the literature and several ideas are given for further research.

Carcinoma gallbladder is the third most common GI malignancy and the most common biliary tract malignancy in North India. Of all the known risk factors, gallstones are most commonly associated with GBC. Bile stasis and chronic inflammation along with occupational exposure to heavy metals, dietary carcinogens, and radiation exposure have been found to be responsible for the increased incidence of GB neoplasms. Infections with S. typhii and S. paratyphii along with Helicobacter sp. have also been associated with GBC. p 53 and ras gene mutations are strongly associated with GBC. Early diagnosis of GBC still remains a challenge and is often diagnosed as a part of investigations of any other disease. Most GBCs are at advanced stage at presentation. USG and CT scan are the common modes for the preliminary diagnosis of GBC. Surgery remains the key treatment modality and the only hope for long-term survival. A large number of patients present at the unresectable stage of the tumor, and palliation remains the only alternative to ensure better quality of life.

Hypoxia, i.e. insufficient availability of oxygen within the tissue, is a more serious problem than realized in clinical routine work, although a large variety of theoretical and experimental data give clearly proven indications. During incidences and progress of many types of diseases hypoxia may enhance development and progress of the disease and, furthermore, decrease chances of efficient treatment of, e.g., tumors. Hypoxic tumor cells are up to three times more resistant towards radiation therapy than normoxic cells. With a diagnostic tool such as provided by hypoxia tracers suitable for PET and SPECT imaging, the particular tissue situation becomes detectable and, thus, treatable. Although the clinical importance of hypoxia imaging is obviously great, to date neither the clinical nor the scientific community at an international level has focused many research activities on the hypoxia problem. As the result of a workshop held by the Section for Radiopharmacy in Tübingen, this book collects observations and findings in order to stimulate the on-going research resulting in important clinical applications. Hypoxia imaging is needed in areas of cardiology, neurology and oncology. Additional applications are to be expected in the field of diabetes, infections and organ transplantation.
