This report investigates undernutrition's persistence, drawing on case studies of the public response to the problem in Ghana, Mozambique, Nigeria, and Uganda. Analyzing each nation's policymaking structures, political actors, understanding of undernutrition, and the timing of public responses, the author explains why none of these four nations has mounted an effective campaign to eliminate undernutrition. The author identifes several different causes of this shortcoming, with one underlying flaw in the various public responses standing out: a fundamental failure on the part of political leaders to see undernutrition as a grave problem that undermines development efforts in their nations. Africa is not on track to meeting the Sustainable Development Goal (SDG) 2 targets to end hunger and ensure access by all people to safe, nutritious and sufficient food all year round and to end all forms of malnutrition. The number of hungry people on the continent has risen by 47.9 million since 2014 and now stands at 250.3 million, or nearly one-fifth of the population. The 2017, 2018 and 2019 editions of this report explain that this gradual deterioration of food security was due to conflict, weather extremes, and economic slowdowns and downturns, often overlapping. A continued worsening of food security is expected also for 2020 as a result of the COVID-19 pandemic. In addition to hunger, across all countries in Africa millions of people suffer from widespread micronutrient deficiencies, and overweight and obesity are emerging as significant health concerns in many countries. This report shows that the food system in Africa does not provide food at a cost that makes nutritious food affordable to a majority of the population, and this is reflected in the high disease burden associated with maternal and child malnutrition, high body-mass, micronutrient deficiencies and dietary risk factors. The report also shows that current food consumption patterns impose high health and environmental costs, which are not reflected in food prices. The findings presented in this report highlight the importance of prioritizing the transformation of food systems to ensure access to affordable and healthy diets for all, produced in a sustainable manner.

Reducing undernutrition requires improving access to goods and services from a wide range of economic and social sectors, including agriculture, education and health. Yet despite broad agreement on the multisectoral nature of the global burden of undernutrition, relatively little research has analyzed how different dimensions of accessibility, such as urbanization and travel times to urban centers, affect child nutrition and dietary outcomes. In this paper we study these relationships in sub-Saharan Africa, a highly rural continent still severely hindered by remoteness problems. We link spatial data on travel times to 20,000 person cities to survey data from 10,900 communities in 23 countries. We document strong negative associations between nutrition indicators and rural livelihoods, but only moderately strong associations with remoteness to cities. Moreover, the harmful effects of remoteness and rural living largely disappear once education, wealth, and social/infrastructural services indicators are added to the model. This implies that the key nutritional disadvantage of rural populations stems chiefly from social and economic poverty. Combating these problems requires either an acceleration of urbanization processes, or finding innovative cost-effective mechanisms for extending basic services to isolated rural communities.

In the 2017 and 2018 editions of the Africa Regional Overview of Food Security and Nutrition, FAO reported that the prevalence of undernourishment was rising in the region. The latest data shows that the deterioration has slowed, but there remain 256 million hungry people in Africa today. The report further documents that although many African countries are making progress towards reducing malnutrition, progress is too slow to meet six key nutrition targets, which form part of the Sustainable Development Goals (SDGs) monitoring framework and the World Health Assembly global nutrition targets. Food insecurity has been rising in Africa in recent years and the continent is not on track to eliminate hunger by 2030. The 2017, 2018 and this year’s report identify and report in detail on conflict, climate extremes and economic slowdowns and downturns as the key drivers of the rise in food insecurity. In most cases, the economic slowdowns and downturns that contributed to rising undernourishment in 2014–2018 were the result of commodity price falls. Many effective policy tools are available, but their adoption will depend on the availability of fiscal space to effect the desired policy action. In the longer-term, countries must develop policies and invest to achieve a more diversified economy and achieve an inclusive structural transformation. However, sustained economic growth is not enough: reducing inequalities, including gender-based and spatial inequalities, is essential to strengthening household resilience, laying the path to inclusive growth and reducing food insecurity and tackling the multiple forms of malnutrition.

Suboptimal diets are a major risk factor for avoidable death and disease in low- and middle-income countries. Evidence shows that some foods or food components (e.g., processed red meat, saturated fat, salt, sugar) significantly elevate the risk of noncommunicable diseases and mortality, while others protect health (e.g., vegetables, fruits, pulses, nuts/seeds, fish, whole grains—referred to in this document as “protective foods”). We used household surveys to compare dietary patterns in Ethiopia, Kenya, Tanzania, and Uganda to the EAT-Lancet healthy reference diet and to quantify and explain consumption gaps for nutritious foods. Compared to the EAT-Lancet healthy reference diet, consumption gaps for pulses and nuts/seeds, vegetables, and fruits are large for both poor and rich consumers in rural and urban areas in the four countries studied, while consumption gaps for meat, fish, and eggs and dairy foods are much larger for lower income groups. Food expenditures of most households in these four countries are far too low to allow consumption of the healthy reference diet; animal-source foods and vegetables are the largest cost components of food expenditures, although quantities consumed of both food groups are much lower than the EAT-Lancet healthy reference diet. Income elasticities of demand for pulses and nuts/seeds and vegetables are often low or moderate, suggesting weak consumer preference for these foods, while income elasticities for fruits and animal-source foods are relatively high. Income growth alone will not solve dietary problems in East Africa; in addition to supply-side interventions to improve affordability, special interventions are required to increase consumer demand for underappreciated protective (nutritious) foods such as pulses and nuts/seeds and vegetables.

Africa is facing a food crisis of unprecedented proportions. Millions are expected to be at risk of worsening hunger in the near future due to the rippling effects of the war in Ukraine, which are compounding the devastating impacts that conflicts, climate variability and extremes, economic slowdowns and downturns, and the aftereffects of the COVID-19 pandemic are having on the most vulnerable. In this context, social and gender inequalities are also on the rise, with women and girls being among the most affected by these shocks. Despite efforts made in several countries, the African continent is not on track to meet the food security and nutrition targets of the Sustainable Development Goal 2 on Zero Hunger for 2030, and certainly the Malabo targets of ending hunger and all forms of malnutrition by 2025. The most recent estimates show that nearly 282 million people in Africa (about 20 percent of the population) were undernourished in 2022, an increase of 57 million people since the COVID-19 pandemic began. About 868 million people were moderately or severely food-insecure and more than one-third of them – 342 million people – were severely food-insecure. The present edition of the report presents the latest analysis of the prevalence and trends in undernourishment, food insecurity, and malnutrition. In addition, it includes, for the first time, estimates of the cost and affordability of a healthy diet, which are useful indicators of people’s economic access to nutritious foods and healthy diets. The deterioration of the food security situation and the lack of progress towards the WHO global nutrition targets make it imperative for countries to step up their efforts if they are to achieve a world without hunger and malnutrition by 2030. The call for greater action remains true in view of the projected lower rate of economic growth, high general and food price inflation, and raising borrowing costs on domestic and international markets since 2022.
