This report investigates undernutrition's persistence, drawing on case studies of the public response to the problem in Ghana, Mozambique, Nigeria, and Uganda. Analyzing each nation's policymaking structures, political actors, understanding of undernutrition, and the timing of public responses, the author explains why none of these four nations has mounted an effective campaign to eliminate undernutrition. The author identifes several different causes of this shortcoming, with one underlying flaw in the various public responses standing out: a fundamental failure on the part of political leaders to see undernutrition as a grave problem that undermines development efforts in their nations. The author concludes that an effective response to undernutrition in these countries requires the formation of national advocacy coalitions that can raise public awareness of the problem, highlight policymakers' duty to ensure the nutrition of their citizens, and link proper nutrition to general national development. Reducing undernutrition requires improving access to goods and services from a wide range of economic and social sectors, including agriculture, education and health. Yet despite broad agreement on the multisectoral nature of the global burden of undernutrition, relatively little research has analyzed how different dimensions of accessibility, such as urbanization and travel times to urban centers, affect child nutrition and dietary outcomes. In this paper we study these relationships in sub-Saharan Africa, a highly rural continent still severely hindered by remoteness problems. We link spatial data on travel times to 20,000 person cities to survey data from 10,900 communities in 23 countries. We document strong negative associations between nutrition indicators and rural livelihoods, but only moderately strong associations with remoteness to cities. Moreover, the harmful effects of remoteness and rural living largely disappear once education, wealth, and social/infrastructural services indicators are added to the model. This implies that the key nutritional disadvantage of rural populations stems chiefly from social and economic poverty. Combating these problems requires either an acceleration of urbanization processes, or finding innovative cost-effective mechanisms for extending basic services to isolated rural communities.

Africa is not on track to meeting the Sustainable Development Goal (SDG) 2 targets to end hunger and ensure access by all people to safe, nutritious and sufficient food all year round and to end all forms of malnutrition. The number of hungry people on the continent has risen by 47.9 million since 2014 and now stands at 250.3 million, or nearly one-fifth of the population. The 2017, 2018 and 2019 editions of this report explain that this gradual deterioration of food security was due to conflict, weather extremes, and economic slowdowns and downturns, often overlapping. A continued worsening of food security is expected also for 2020 as a result of the COVID-19 pandemic. In addition to hunger, across all countries in Africa millions of people suffer from widespread micronutrient deficiencies, and overweight and obesity are emerging as significant health concerns in many countries. This report shows that the food system in Africa does not provide food at a cost that makes nutritious food affordable to a majority of the population, and this is reflected in the high disease burden associated with maternal and child malnutrition, high body-mass, micronutrient deficiencies and dietary risk factors. The report also shows that current food consumption patterns impose high health and environmental costs, which are not reflected in food prices. The findings presented in this report highlight the importance of prioritizing the transformation of food systems to ensure access to affordable and healthy diets for all, produced in a sustainable manner.

The 2016 Global Hunger Index Africa Edition is produced by the International Food Policy Research Institute (IFPRI), Concern Worldwide, and Welthungerhilfe in conjunction with NEPAD. The GHI Africa Edition is based on data taken from the 2016 Global Hunger Index. IFPRI has calculated the Global Hunger Index, a multidimensional measure of global hunger, for the past eleven years. The series tracks the state of hunger across the globe and spotlights those regions and countries where action to address hunger is most urgently needed. The GHI Africa Edition shows that although progress between countries varies, the level of hunger in all countries across the continent of Africa, for which GHI scores could be calculated, has declined since 2000. Despite this progress, the level of hunger in many countries remains unacceptably high. Of the 42 countries in Africa for which GHI scores could be calculated in this report, only three countries have scores that fall into the “low” hunger category, while 28 fall into the “serious” category and five countries have 2016 scores in the “alarming” category. It is clear that governments must now follow through on their promises with political will and commitment to action that is both strong and sustained. The root causes of hunger are complex and require multi-sectoral and multilevel collaboration. The role of national governments in achieving these goals by significantly enhancing the quality of implementation is also clear. Yet Zero Hunger can only be achieved when governments measure progress and are accountable to citizens, which requires capacities to collect and analyze data, combined with open and comprehensive review and dialogue processes. The biennial review process established under Malabo and the support to inclusive Joint Sector Review (JSR) processes under CAADP are critical building blocks in this regard.

Africa is facing a food crisis of unprecedented proportions. Millions are expected to be at risk of worsening hunger in the near future due to the rippling effects of the war in Ukraine, which are compounding the devastating impacts that conflicts, climate variability and extremes, economic slowdowns and downturns, and the aftereffects of the COVID-19 pandemic are having on the most vulnerable. In this context, social and gender inequalities are also on the rise, with women and girls being among the most affected by these shocks. Despite efforts made in several countries, the African continent is not on track to meet the food security and nutrition targets of the Sustainable Development Goal 2 on Zero Hunger for 2030, and certainly the Malabo targets of ending hunger and all forms of malnutrition by 2025. The most recent estimates show that nearly 282 million people in Africa (about 20 percent of the population) were undernourished in 2022, an increase of 57 million people since the COVID-19 pandemic began. About 868 million people were moderately or severely food-insecure and more than one-third of them – 342 million people – were severely food-insecure. The present edition of the report presents the latest analysis of the prevalence and trends in undernourishment, food insecurity, and malnutrition. In addition, it includes, for the first time, estimates of the cost and affordability of a healthy diet, which are useful indicators of people’s economic access to nutritious foods and healthy diets. The deterioration of the food security situation and the lack of progress towards the WHO global nutrition targets make it imperative for countries to step up their efforts if they are to achieve a world without hunger and malnutrition by 2030. The call for greater action remains true in view of the projected lower rate of economic growth, high general and food price inflation, and raising borrowing costs on domestic and international markets since 2022.
